THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 7 January 2008, be corrected to show the dates he received imminent danger pay, deployment dates, and any medals which he is entitled to receive.
2. The applicant does not add any statement to his request.
3. The applicant provides a copy of his DD Form 214 and a certificate which shows he was awarded the Army Achievement Medal for the period 8 August to 3 September 2005.
CONSIDERATION OF EVIDENCE:
1. The applicant's military records show that he enlisted in the Regular Army on 6 April 2004, and he was awarded the military occupational specialty of health care specialist.
2. The applicant was honorably released from active duty, on 7 January 2008, in the rank of specialist, pay grade E-4.
3. In the processing of this case the Army Review Boards Agency contacted the Defense Finance and Accounting Service (DFAS) who confirmed that the applicant received imminent danger pay for Kuwait from February to November 2006.
4. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. In pertinent part, it directs that when an active duty Soldier is deployed with his or her unit during the period covered by the DD Form 214, an entry will be made in Item 18 to show "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD YYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. DFAS records show the applicant received imminent danger pay for Kuwait from February to November 2006.
2. There are no other records to show the dates the applicant was deployed to Kuwait.
3. Imminent danger pay is given in full months. Since deployed service entries on a DD Form 214 are exact dates, and there is insufficient evidence showing the exact dates the applicant served in Iraq, there is insufficient documentation in which to grant the applicant's request.
4. Since there is no evidence to show the applicant was deployed to Iraq, there is insufficient evidence in which to base award of any service medals.
