THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that the Army Achievement Medal (AAM) Certificate belonging to Sergeant N----- L. N-----, for exceptionally meritorious achievement during the period 25 June 2007 to 24 November 2007, be removed from his Official Military Personnel File (OMPF).

2.  The applicant states that his Social Security Number (SSN) was mistakenly placed on his squad leader’s DA Form 638 (Recommendation for Award) and AAM Certificate, and placed in his OMPF.  

3.  The applicant provides a copy of Sergeant N-----'s DA Form 638, dated
29 December 2007, with the applicant’s SSN on it; and Sergeant N-----'s AAM Certificate, with the applicant’s SSN on it, in support of his application. 

CONSIDERATION OF EVIDENCE:

1.  The applicant is currently serving on active duty in the rank of sergeant.

2.  A review of the applicant’s OMPF on iPERMS revealed a copy of the AAM Certificate that was awarded to Sergeant N-----. 

DISCUSSION AND CONCLUSIONS:

The AAM Certificate in question undoubtedly belongs to another Soldier as evidenced by the corresponding DA Form 638 of the same name provided by the applicant.  Although the DA Form 638 and AAM Certificate contains the applicant's SSN, he acknowledges the award belongs to the other Soldier.  Therefore, the AAM Certificate should be removed from the applicant's records and refiled in the appropriate records of the other Soldier, with the correction of the SSN.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by removing from the applicant's iPERMS records the AAM Certificate belonging to Sergeant N-----, and refiling the award, with the correct SSN, in the appropriate records.
