THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that item 14 (Military Education) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show she completed basic training. She also requests that her last name be changed on her military records.
2. The applicant states, in effect, that she attended basic training during the period 21 February 2007 to 20 April 2007; however, item 14 of her DD Form 214 does not reflect this training. She also states that she has remarried and would like her records to show her current last name.
3. The applicant provides a copy of her DD Form 214; a memorandum for record, dated 29 August 2008; an enlistment contract; and a marriage license in support of her application.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 21 February 2007 under the last name R____. On 20 April 2007, she was discharged with an uncharacterized discharge under the provisions of Army Regulation 635-200 (Active Duty Enlisted Administrative Separations), paragraph 5-11, for failure to meet procurement medical fitness standards. She had served a total of 2 months of creditable active service.
2. The applicants DD Form 214 shows the last name R____. Item 14 on her DD Form 214 shows the entry, "NONE//NOTHING FOLLOWS."
3. In support of her claim to amend item 14 of her DD Form 214, the applicant provided a memorandum for record, dated 29 August 2008, from her company commander at the time in question. He attests that the applicant completed basic training.
4. In support of her claim to change her last name, the applicant provided a marriage license which shows she was married on 24 December 2007 and her last name was changed to B____.
5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part it states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. The regulation also provides that item 14 will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by title, length in weeks, and year completed. Training courses for combat skills will not be listed.
DISCUSSION AND CONCLUSIONS:
1. Evidence of record shows the applicant completed basic training. However, since the governing regulation states that training courses for combat skills will not be listed on the DD Form 214, there is no basis for granting the applicants request to show this training course in item 14 on her DD Form 214.
2. For historical purposes, the Army has an interest in maintaining the accuracy of its records. The data and information contained in those records should actually reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, this Board is reluctant to recommend that those records be changed.
3. The applicants enlistment contract shows the last name R____. It appears she appropriately served and was discharged under the last name R____. She did not change her last name until after her separation. While the Board understands the applicant's desire to have the records changed, it finds no basis for compromising the integrity of the Army's records, in particular, the applicants DD Form 214. This Board action will be filed in the applicants military records so a record of her current married name (B____) will be on hand.
