THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show he was awarded a third  Meritorious Service Medal (3rd Award) but only four awards of the Army Commendation Medal (4th award).

2.  The applicant states the proper certificates were not available at the time his DD Form 214 was prepared.  

3.  The applicant provides copies of his DD Form 214; three Meritorious Service Medal (MSM) certificates; and four Army Commendation Medal (ARCOM) certificates in support of his application.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 26 November 1986.  He was honorably discharged on 28 February 1990 for the purpose of accepting a commission as an officer.

2.  The applicant was commissioned a second lieutenant and entered active duty in the Quartermaster Corps on 1 March 1990.  He honorably served until 30 November 2008 at which time he was retired by reason of sufficient service for retirement.

3.  Item 13 of the applicant's DD Form 214 for the period ending 30 November 2008 shows the "Meritorious Service Medal (2nd award)" and the "Army Commendation Medal (5th Award)."

4.  The applicant's Officer Record Brief shows the entry "MSM - 1" and
"ARCOM - 5."

5.  The remainder of the applicant's awards are correctly documented on the DD Form 214 and in his official military records.

6.  The available record does not contain any separate orders for any of his Meritorious Service Medals or Army Commendation Medals.  However, it does contain award certificates for these medals identical to those provided by the applicant. 

7.  The certificates in the official record show award of the Meritorious Service Medal as follows: 

	a.  on 25 August 2008, for the period from 1 December 1998 to 30 November 2008, in accordance with U.S. Army Training and Doctrine Command, Fort Monroe, Virginia, Permanent Order 238-4, dated 25 August 2008; 

	b.  on 22 May 2006, for the period from 15 January 2003 to 30 June 2006, in accordance with Headquarters, USAMANSCEN, Fort Leonard Wood, Missouri, Permanent Order Number 150-14, dated 30 May 2006; and 

	c.  on 13 December 2002, for the period from 1 January 1998 to 31 December 2002, in accordance with Headquarters, USAARMC, Fort Knox, Kentucky, Permanent Order Number 347-00001, dated 13 December 2002.

8.  The certificates in the official record show award of the Army Commendation Medal as follows:

	a.  on 25 August 1997 for the period from 10 April 1994 to 28 August 1997, in accordance with 10th Mountain Division, Fort Drum, New York, Permanent Orders 237-1;

	b.  on 25 August 1993, for the period from 16 September 1992 to 27 September 1993, in accordance with Headquarters, 16th Corps Support Group, Hanau, Germany, Permanent Orders 049-5;
 
	c.  on (date omitted), for the period from 17 January 1991 to 28 February 1991, in accordance with 26th Support Battalion, First Armored Division Support Command Permanent Order Number 26-96; and 
	
   d.  on 3 June 1988, for his actions on 27 April 1988 while serving with the Combat Support Company, 3rd Battalion, 47th Infantry Regiment.  No order number is listed on the certificate.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends his last DD Form 214 does not properly reflect the correct number of awards of the Meritorious Service Medal (3rd Award) and the Army Commendation Medal (4th Award). 

2.  The applicant's submitted award certificates and the available records support the applicant's contention that his DD Form 214 incorrectly omits one award of the Meritorious Service Medal and adds an additional award of the Army Commendation Medal.  Therefore, it would be appropriate to correct the applicant's record to reflect the correct number of Meritorious Service Medals and Army Commendation Medals earned and awarded.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

   a.  deleting from item 13 of the applicant DD Form 214 the entries "Meritorious Service Medal (2nd Award)" and "Army Commendation Medal (5th Award)"; and
   
   
   
   b.  adding to item 13 of the applicant's DD Form 214 the entries "Meritorious Service Medal (3rd Award)" and "Army Commendation Medal (4th Award)."
