THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that the dates of his deployment for Operation Iraqi Freedom be shown on his DD Form 214 (Certificate of Release or Discharge from Active Duty).
2. The applicant states, in effect, that without the exact deployment dates being shown on his DD Form 214, he may be disqualified from certain benefits. He served in Iraq from 16 January 2006 to 18 February 2007.
3. The applicant provides a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. On 29 May 2007, the applicant was separated from the Regular Army at the completion of his required active duty service and transferred to the United States Army Reserve as a specialist, pay grade E-4. He had completed 3 years, 7 months, and 9 days of active duty service.
2. The Defense Finance and Accounting Service reported in an email message to a member of the staff of the Army Review Boards Agency that the applicant had been authorized imminent danger pay/hostile fire pay from January 2006 through February 2007.
3. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in block 18.
DISCUSSION AND CONCLUSIONS:
1. The applicant states that he served in Iraq from 16 January 2006 to
18 February 2007.
2. The DFAS reported that the applicant had been authorized imminent danger pay/hostile fire pay from January 2006 through February 2007.
3. Block 18 (Remarks) of the applicant's DD Form 214 should be corrected by showing "SERVICE IN IRAQ FROM 20060116 - 20070218."

BOARD DETERMINATION/RECOMMENDATION:
