THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his service in Iraq.
2. The applicant states that this information was omitted from his DD Form 214.
3. The applicant provides a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant served in the Regular Army from 25 July 2000 through
28 December 2007. He was trained in, awarded, and served in military occupational specialty (MOS) 68W (Health Care Specialist). His DD Form 214 shows that he was discharged under the provisions of Army Regulation 635-200, chapter 14-12b, for a pattern of misconduct. His service was characterized as general, under honorable conditions.
2. The applicant served in Iraq from 1 April 2003 30 July 2004 as verified through the receipt of hostile fire pay/imminent danger pay (HFP/IDP) from the Defense Finance and Accounting Service (DFAS), Army Military Pay Operations. This foreign service is not shown on his DD Form 214.
3. Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents (DD Form 214/5). Item 18 (Remarks) lists the name of the country to which deployed and the inclusive dates of service in the country. Each entry is verified by the individual's service record.
DISCUSSION AND CONCLUSIONS:
1. The applicant's DD Form 214 does not show his service performed in Iraq.
2. The applicant's record, as verified by DFAS, shows that he served in Iraq from 1 April 2003 30 July 2004. Therefore, his record should be corrected as directed below.
