THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction to his DD Form 214 (Certificate of Release or Discharge from Active Duty), Item 13 (Decorations, Medal, Badges, Citations, and Campaign Ribbons Awarded or Authorized) to show award of the Army Commendation Medal (2nd Award); Item 14 (Military Education) to show he completed the Javelin Course; the 1st Battalion, 26th Infantry Regiment Team Leader Course; Infantry Basic Training; German Headstart (60 Hours) and correction of Item 18 (Remarks) to show he was deployed to Iraq from 2004 to 2005.
2. The applicant states, in effect, that the following items are missing from his DD Form 214: a period of deployment, training courses, and one award of the Army Commendation Medal. He also states that his DD Form 214 shows he completed 1 week of the German Headstart, but his certificate states he completed 60 hours of the training.
3. In support of his application, the applicant provides copies of his 2004 deployment orders, his training certificates, and his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant's military record shows he enlisted in the Regular Army, in pay grade E-1, on 24 April 2003. He completed basic and advanced training and was awarded military occupational specialty 11B, Infantryman.
2. The applicant submitted a diploma, dated 8 August 2003, for completion of Infantry Basic Training from 2 May to 8 August 2003; a diploma, dated 22 August 2003 for completion of the Javelin Course; a certificate, dated 24 October 2003, for completion of ITC Training and German Headstart (60 Hours), and a certificate for completion of the 1st Battalion, 26th Infantry Regiment Team Leader Course (80 Hours), on an unknown date.
3. The applicants Enlisted Record Brief, Section VI, Military Education, contains no entries which correspond to the requested entries. Section IX, Assignment Information, shows he was stationed in Germany from 29 September 2003 to an unknown date and again from 1 December 2005 to an unknown date.
4. The applicant was issued Order Number TFD 4604, dated 14 January 2004, for a temporary change of station from Germany to CENTCOM (Central Command) AOR (Area of Responsibility) in support of Operation Iraqi Freedom II Rotation, with an effective date of 10 February 2004, for 355 days. The order listed travel authorization to Iraq, Kuwait, Egypt, UAE, Qatar, Oman, Saudi Arabia, Bahrain, Pakistan, Afghanistan, Uzbekistan, Turkey, and Jordan.
5. The applicant also submits a copy of his certificate, dated 25 February 2007, for award of the Army Commendation Medal, for meritorious service during Operation Iraqi Freedom, from 1 September 2006 to 20 June 2007. He further submits a copy of his Certificate of Recognition in support of the Global War on Terror, dated 1 November 2006.
6. A review of the applicant's records failed to reveal orders or other evidence authorizing award of a second Army Commendation Medal during his period of service. The applicant provided no orders to show he was awarded a second Army Commendation Medal.
7. The applicant was honorably discharged from active duty, in pay grade E-4, on 28 February 2008. He was issued a DD Form 214 that shows in Item 12.f. (Foreign Service) a credit of 4 years and 5 months.
8. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), of the applicants DD Form 214, lists the following awards: the Army Commendation Medal; the Army Achievement Medal; the Valorous Unit Award; the Army Good Conduct Medal; the National Defense Service Medal; the Global War on Terrorism Expeditionary Medal; the Global War on Terrorism Service Medal; the Iraq Campaign Medal; the Army Service Ribbon; the Overseas Service Ribbon (2nd Award); and the Combat Infantryman Badge. Item 14 (Military Education) shows he completed 1 week of German Headstart in 2003.
9. Item 18 (Remarks), of the applicants DD Form 214, shows Service in Iraq from 5 August 2006 to 1 October 2007, a period of 1 year, 1 month, and 27 days.
10. A staff member of the Defense Finance and Accounting Service, Indianapolis, Indiana, verified that the applicant served in Kuwait from February 2004 to February 2005.
11. Army Regulation 635-5 governs the preparation of the DD Form 214. It states, in pertinent part, that Foreign Service performed during the period covered by the DD Form 214 will be entered in item 12f. The regulation also states that the amount of foreign service will be taken from the Soldiers Enlisted Qualification Record or, if necessary, verified from the Soldiers Military Personnel Records Jacket. However, for active duty Soldiers deployed with their unit during their continuous period of active service, the entry "SERVICE IN (NAME OF COUNTRY DEPLOYED) from (Inclusive dates for example, YYYYMMDD-YYYYMMDD) will be entered in Item 18 of the DD Form 214.
12. Army Regulation 635-5 also specifies that Item 14 would list in-service training courses successfully completed during the period covered by the DD Form 214 prepared. The list will include the title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not listed.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record confirms the applicant served in Kuwait from February 2004 to February 2005, a period of 12 months. It appears that Item 12.f., of his DD Form 214, includes foreign service for his assignments in Germany, Kuwait, and Iraq. However, "Service in Kuwait from 200402 to 200502 is not shown in Item 18 of his DD Form 214, therefore, it should be corrected to show this foreign service.
2. In accordance with regulatory guidance, courses listed on the DD Form 214 will include the title, length in weeks, and year completed. The evidence shows the applicant attended German Headstart from 7 to 24 October 2003, a period equating to approximately two weeks. Therefore, it is concluded he is entitled to correction of his DD Form 214, Item 14, to show completion of German Headstart, 2 weeks, 24 October 2003.
3. In accordance with regulatory guidance, his completion of such training courses for combat skills as the the Javelin Combat Arms Course; Infantry Basic Training; and the 1st Battalion, 26th Infantry Regiment Team Leader Course are not authorized to be listed in Item 14 of the DD Form 214.
4. The available evidence does not confirm and the applicant did not provide any evidence to show that he is authorized a second award of the Army Commendation Medal. Therefore, the applicant is not entitled to correction of his DD Form 214 to reflect a second award of the Army Commendation Medal.
5. In view of the foregoing, the applicants records should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. amending the applicants DD Form 214, Item 18 to show "Service in Kuwait from 200402 to 200502"; and
b. amending the DD Form 214, Item 14 to show German Headstart, 2 weeks, 2003, and by providing him a corrected separation document that includes these corrections.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to correction to his DD Form 214 to show award of the Army Commendation Medal (2nd Award); completion of Infantry Basic Training; the Javelin Combat Arms Course; and the 1st Battalion, 26th Infantry Regiment Team Leader Course.
