	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his records to show he was awarded an additional Army Commendation Medal, an additional Good Conduct Medal, a North Atlantic Treaty Organization (NATO) Medal, and an Armed Forces Expeditionary Medal (AFEM).  He also requests correction of his records to show his overseas tours in Germany, Bosnia-Herzegovina, and Hawaii.

2.  The applicant states, in effect, that the omissions from his record may be the result of his transition from the enlisted ranks to the warrant officer ranks.

3.  The applicant provides copies of his DD Form 214 covering the period
11 January 1994 through 2 July 2003, an Army Commendation Medal Certificate, orders awarding the Good Conduct Medal (1st Award), and a memorandum authorizing wear of the NATO Medal and Armed Forces Expeditionary Medal as documentary evidence in support of this application.

CONSIDERATION OF EVIDENCE:

1.  The applicant's record includes a DD Form 214 covering the period
10 October 1989 through 28 June 1993 that shows he possessed the military occupational specialty of 63B (Light Wheel Vehicle Mechanic) and the rank of specialist/pay grade E-4 at the time he was released from active duty with an honorable characterization of service.  Following a six-month break in service, the applicant enlisted into the Army on 11 January 1994.  He completed basic combat training and advanced individual training.  Upon completion of advanced individual training, he was awarded the military occupational specialty 63H (Track Vehicle Repairer).  The highest enlisted rank he attained prior to converting to warrant officer was staff sergeant/pay grade E-6.  The applicant completed the warrant officer candidate course and the warrant officer basic course.  Upon completion of the warrant officer basic course, he was awarded the military occupational specialty of 915A (Unit Maintenance Technician).  The applicant is currently a chief warrant officer two/pay grade W-2 serving on active duty at Fort Irwin, California.

2.  The applicant provides a copy of a certificate for the Army Commendation Medal presented to him for meritorious achievement for the period 11 October 2001 to 16 September 2002.  The certificate was issued under the authority of 
Multi-National Division (North) and Task Force Eagle, Bosnia-Herzegovina, Permanent Orders Number A229-021, dated 17 August 2002.

3.  Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 covering the period of 
11 January 1994 through 2 July 2003 shows among the listed awards one Army Commendation Medal.

4.  Further review of the applicant's Official Military Personnel File shows award of two Army Commendation Medals during this time period as follows:

	a.  Multi-National Division (North) and Task Force Eagle, Bosnia-Herzegovina, Permanent Orders Number A229-021, dated 17 August 2002; and

	b.  Division Support Command, Schofield Barracks, Hawaii, Permanent Orders Number 028-01, dated 28 January 2003.

5.  The applicant provides a copy of 2nd Battalion, 3rd Aviation Regiment, 11th Aviation Brigade, Army Europe Permanent Orders 212-10-7, dated 10 December 1992, awarding him the Good Conduct Medal (1st Award) for the period of
10 October 1989 through 9 October 1992.

6.  Item 13 of the applicant's DD Form 214 covering the period of 10 October 1989 through 28 June 1993 shows among the listed awards one Army Good Conduct Medal.

7.  The applicant provides a copy of a memorandum from 225th Forward Logistics Element, 25th Infantry Division (Light), Tuzla, Bosnia-Herzegovina, dated 15 July 2002, authorizing him to wear the North Atlantic Treaty Organization (NATO) Medal and Armed Forces Expeditionary Medal (AFEM) awarded to him for service in support of Operation Joint Forge in Bosnia-Herzegovina during the period February through October 2002.

8.  There is no copy of the aforementioned memorandum in the applicant's record and item 13 of the applicant's DD Form 214 covering the period
11 January 1994 through 2 July 2003 does not show award of the North Atlantic Treaty Organization Medal or the Armed Forces Expeditionary Medal.

9.  Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the North Atlantic Treaty Organization (NATO) Medal was authorized by the Secretary General of the North Atlantic Treaty Organization for specific NATO 
operations.  The Secretary of Defense may approve acceptance and wear by United States service members who meet the criteria specified by the Secretary General of NATO.  Acceptance of the NATO Medal has been approved for U.S. 

military personnel who serve under NATO command or operational control in direct support of NATO operations in the former Republic of Yugoslavia from 1 July 1992 through a date to be determined.

10.  Army Regulation 600-8-22 provides, in pertinent part, that the Armed Forces Expeditionary Medal is authorized for participants in military operations within a specific geographic area during a specified time period.  The regulation also provides that participants in Operation Joint Forge located in the former Republic of Yugoslavia are authorized this award.

11.  DA Form 2166-8 (NCO [Noncommissioned officer] Evaluation Report), dated, 1 September 2002, covering the period December 2001 through September 2002, shows the applicant deployed to Bosnia-Herzegovina in Support of Operation Joint Forge and served as a member of Task Force Eagle.

12.  Table 2-1 (Composition of the Official Military Personnel File) of Army Regulation 600-8-104 (Military Personnel Information Management/Records), in pertinent part, indicates that documents and certificates that award badges, service medals, tabs or non-Army awards for which no orders are published should be filed in the Commendatory and Disciplinary section of the Service portion of the Official Military Personnel File.

13.  Item 8a (Last Duty Assignment and Major Command) of the applicant's DD Form 214 covering the period 10 October 1989 through 28 June 1993 shows he was assigned to Headquarters and Headquarters Company, 2nd Battalion, 3rd Aviation Regiment, located in United States Army Europe (USAREUR).  Item 13 of the applicant's DD Form 214 shows award of the Overseas Service Ribbon.

14.  Three DA Forms 2166-8 covering the period June 1999 through September 2002 show the applicant was assigned to the 225th Forward Support Battalion of the 25th Infantry Division (Light) located at Schofield Barracks, Hawaii.  Item 13 of the applicant's DD Form 214 covering the period 11 January 1994 through
2 July 2003 shows award of the Overseas Service Ribbon.

15.  Table 4-1 (Update Procedures for Officer Record Brief) of Army Regulation 600-8-104, in pertinent part, indicates that for officers and warrant officers with prior enlisted service, overseas tours as enlisted will be shown in Section I (Assignment Information).  All overseas service performed by authority of the Department of Defense will be recorded in this item.


16.  Review of the applicant's records indicates entitlement to additional awards and decorations that are not shown on his DD Form 214.

17.  Item 13 of the applicant's DD Form 214, covering the period 11 January 1994 through 2 July 2003 shows award of one Army Good Conduct Medal.  Further review of the applicant's Official Military Personnel File shows award of three Army Good Conduct Medals during this time period as follows:

	a.  Bravo Detachment, 15th Personnel Services Battalion, Fort Hood, Texas, Permanent Order 226-211, dated 14 August 1997, awarded the Good Conduct Medal (2nd Award) for the period 11 January 1994 to 10 January 1997;

	b.  Bravo Detachment, 556th Personnel Services Battalion, Schofield Barrack, Hawaii, Permanent Orders 63-323, dated 3 March 2000, awarded the Good Conduct Medal (3rd Award) for the period of 11 January 1997 to
10 January 2000; and

	c.  Bravo Detachment, 556th Personnel Services Battalion, Schofield Barrack, Hawaii, Permanent Order B010-23, dated 10 January 2003, awarded the Good Conduct Medal (4th Award) for the period 11 January 2000 to
10 January 2003.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that his records should be corrected to show he was awarded an additional Army Commendation Medal, an additional Good Conduct Medal, a North Atlantic Treaty Organization Medal, an Armed Forces Expeditionary Medal, and credit for his overseas tours in Germany, Bosnia-Herzegovina, and Hawaii was carefully considered and determined to be with merit.

2.  Permanent Orders awarded the applicant a total of two Army Commendation Medals.  Therefore, the applicant is entitled to have his records corrected to show Army Commendation Medal (2nd Award).

3.  Permanent Orders awarded the applicant a total of four Army Good Conduct Medals.  Therefore, the applicant is entitled to have his records corrected to show Army Good conduct Medal (4th Award).



4.  Army Regulation 600-8-22 provides, in pertinent part, that the applicant was authorized to accept award of the North Atlantic Treaty Organization (NATO) 
Medal for his participation Operation Joint Forge while located in the former Republic of Yugoslavia.  Therefore, the applicant is entitled to have his records corrected to show the North Atlantic Treaty Organization Medal.

5.  Army Regulation 600-8-22 provides, in pertinent part, that the applicant is authorized the Armed Forces Expeditionary Medal for his participation in Operation Joint Forge while located in the former Republic of Yugoslavia.  Therefore, the applicant is entitled to have his records corrected to show the Armed Forces Expeditionary Medal.

6.  Records show the applicant was awarded the Overseas Service Ribbon for his tours of service in Hawaii and the Federal Republic of Germany.  Therefore, the applicant is entitled to have these overseas tours added to his Officer Record Brief.

7.  Records show the applicant was deployed to Bosnia-Herzegovina for the period 4 March 2002 through 15 September 2002.  Therefore, the applicant is entitled to have this deployment added to his records.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected to show Army Commendation Medals (3rd Award), Army Good Conduct Medal
(4th Award), the North Atlantic Treaty Organization Medal, and the Armed Forces Expeditionary Medal.



2.  The Board further determined that the evidence presented was sufficient to warrant addition of the applicant's overseas tours of duty in Hawaii, the Federal Republic of Germany, and Bosnia-Herzegovina to his official records.
