The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion, if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal with "V" Device.

2. The applicant states that this award was not stated on his DD Form 214 upon separation.

3. The applicant provides a copy of the DA Form 638 (Recommendation for Award) in support of his application.

CONSIDERATION OF EVIDENCE:

1. The applicant's records show that, as a member of the Pennsylvania Army National Guard (PAARNG), he was ordered to active duty on 25 January 2005 in support of Operation Iraqi Freedom. Records further show that the applicant served in Iraq during the period 27 June 2005 through 10 June 2006. He was honorably released from active duty on 8 July 2006.

2. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 does not show award of the Army Commendation Medal with "V" Device.

3. The applicant's DA Form 2-1 (Personnel Qualification Records) is not available for review.

4. Headquarters, 2nd Battalion, 28th Brigade Combat Team (BCT) Orders Number 078-004, dated 19 March 2006, show the applicant was awarded the Army Commendation Medal with "V" Device for heroism in connection with military operations against a hostile force in Iraq during Operation Iraqi Freedom while assigned as a forward observer, C Company, 1st Battalion, 172nd Armor, on 1 March 2006.

DISCUSSION AND CONCLUSIONS:

Orders show the applicant was awarded the Army Commendation Medal with "V" Device for heroism in connection with military operations against a hostile force in Iraq on 1 March 2006. Therefore, he is entitled to correction of his records to show this award.
