THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show her correct date entered active duty for this period and the correct net active duty service for this period of active duty.
2. The applicant states the dates of service on her DD Form 214 should be shown as 7 November 2003 to 22 July 2004.
3. The applicant provides an unsigned DD Form 214 Worksheet and her Army Basic Training School Reservation Form dated 2 September 2003.
CONSIDERATION OF EVIDENCE:
1. The applicant entered active duty on 6 November 2003. She was separated on 22 July 2004 for a physical condition, not a disability under the provisions of Army Regulation 635-200 (Personnel Separations), paragraph 5-17.
2. The DD Form 214 issued at the time of the applicant's discharge shows in Block 12a (Date Entered AD This Period) the entry "2004 07 22" and in Block 12b (Separation Date This Period) the entry "2004 07 22."
3. The applicant's personnel service record shows she enlisted in the Regular Army on 6 November 2003.
4. The applicant provided a copy of DD Form 214 Worksheet unsigned and undated which shows Block 12a (Date Entered AD This Period) the entry "2004 07 22" and in Block 12b (Separation Date This Period) the entry "2004 07 22". The applicant also provided a copy of her Army training reservation receipt dated 2 September 2003 for Army Basic Training and Advanced Individual Training which shows the start date of 14 November 2003.
5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. Paragraph 2-1 of this regulation states that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge.
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that her DD Form 214 incorrectly shows her record of service was carefully considered and determined to have merit.
2. The applicant's personnel service records show she entered active duty on 6 November 2003 and that she was separated from active duty on 22 July 2004 with an honorable characterization of service.
3. Based on the foregoing information, the applicant's DD Form 214 should be amended to show her date entered active duty as 6 November 2003, her separation date as 22 July 2004, and her net active service for this period as 8 months and 17 days.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the DD Form 214 to show the entry in Block 12a (Date Entered AD This Period) "2003 11 06", Block 12b (Separation Date This Period) "2004 07 22", and Block 12c (Net Active Service This Period) "0000 08 17".
