THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Global War on Terrorism Expeditionary Medal (GWOTEM) and certificates of recommendation and commendation. She also requests that item 14 (Military Education) be corrected to show she completed Financial Readiness Training and the S1/SIDPERS Workshop; and that item 18 (Remarks) on her DD Form 214 be corrected to show a deployment.
2. The applicant states, in effect, that she works for veterans and reviews their DD Forms 214. She contends that she has found that a lot of the veterans DD Forms 214 show letters of recommendation and certificates of achievement in the awards section. She also states that Operation Noble Eagle was a deployment under Operation Enduring Freedom and that the Expeditionary Medal should be recognized for this deployment.
3. The applicant provides a copy of her DD Form 214; certificates of achievement and training (8 hour course); an award certificate for the Medal of Excellence; a letter of commendation; a letter of recommendation; deployment orders; and orders for the Joint Service Achievement Medal.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted on 17 October 2001, served as an administrative specialist, and was honorably discharged on 7 July 2004 by reason of physical disability (severance pay).
2. The applicants DD Form 214 shows the Joint Service Achievement Medal, the Army Achievement Medal (2d Award), the National Defense Service Medal, the Global War on Terrorism Service Medal, the Army Lapel Button, and the Army Service Ribbon as authorized awards. Item 14 on her DD Form 214 shows she completed the Administrative Specialist Course and the Combat Lifesaver Course. Item 18 on her DD Form 214 does not show any deployments.
3. In support of her claim to correct item 14 on her DD Form 214, the applicant provided a certificate of achievement for completion of the S1/SIDPERS Workshop during the period 13-14 May 2003. She also provided a certificate of training for the Financial Readiness Training course (8 hours).
4. In support of her claim to correction of item 18 on her DD Form 214, she provided deployment orders which show she was deployed from Fort Carson, Colorado to Washington, D.C. in support of Operation Enduring Freedom on or about 4 April 2002 to on or about 30 September 2002. She also provided orders for the Joint Service Achievement Medal during the period April 2002 to September 2002.
5. There is no evidence of record which shows that the applicant was deployed in support of Operation Noble Eagle (homeland defense and civil support after
11 September 2001).
6. Army Regulation 600-8-22 (Military Awards) states that the GWOTEM is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in the Global War on Terrorism Operations on or after 11 September 2001 to a date to be determined.
7. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. The regulation provides that item 13 of the DD Form 214 will reflect decorations, medals, badges, citations and campaign ribbons awarded or authorized for all periods of service. Certificates of achievement, letters of appreciation, and similar documents are not recorded on the DD Form 214.
8. Army Regulation 635-5 states, in pertinent part, that item 14 will list formal
in-service training courses of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed.
9. Army Regulation 635-5 states, in pertinent part, that the country and dates of deployment for an active duty Soldier deployed with his or her unit during their continuous period of active service will be shown in item 18 on the DD Form 214.
DISCUSSION AND CONCLUSIONS:
1. There is no evidence of record which shows the applicant deployed abroad for service in the Global War on Terrorism Operations on or after 11 September 2001. Therefore, there is insufficient evidence on which to base award of the GWOTEM in this case.
2. The governing regulation states that certificates of achievement, letters of appreciation, and similar documents are not recorded on the DD Form 214. Therefore, there is no basis for granting the applicants request to show certificates of recommendation and commendation on her DD Form 214.
3. The applicant provided documentation which shows she completed an 8-hour training course and a 16-hour training course. The governing regulation states that item 14 will list formal training courses of 40 hours or more. Therefore, there is no basis for granting the applicants request to show these training courses in item 14 on her DD Form 214.
4. There is no evidence of record which shows that the applicant was deployed in support of Operation Noble Eagle. Orders show the applicant was deployed from Fort Carson, Colorado to Washington, D.C. in support of Operation Enduring Freedom (although since she stayed stateside, the Operation was most likely Noble Eagle) on or about 4 April 2002 to on or about 30 September 2002. As an exception to policy, item 18 on her DD Form 214 will be corrected to show this deployment.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the entry, Deployment from Fort Carson, Colorado to Washington, D.C. during the period 4 April 2002 to 30 September 2002 in item 18 on her DD Form 214.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to award of the GWOTEM; amending item 13 on her DD Form 214 to add certificates of recommendation and commendation; amending item 14 on her DD Form 214; and amending item 18 on her DD Form 214 to show she was deployed in support of Operation Noble Eagle.
