	The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that his dates of service in Iraq be shown on his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states, in effect, that his DD Form 214 does not show his combat service in Iraq.  

3.  The applicant provides a copy of his DD Form 214; deployment orders; and a roster.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted on 18 May 1984 and served as a vehicle maintenance supervisor.  He remained on active duty through continuous reenlistments.  On 31 May 2005, the applicant retired by reason of permanent disability in the rank of sergeant first class after completing over 21 years of creditable active service.

2.  Item 18 (Remarks) on his DD Form 214 does not show any deployments.  His DD Form 214 shows the Global War on Terrorism Service Medal as an authorized award.

3.  The applicant provided documentation which shows he was deployed to the CENTCOM area of responsibility in support of Operation Enduring Freedom in February 2003.  He also provided a roster which shows his name and his base camp as Tikrit, Iraq.  However, his inclusive dates of service in Iraq are not available.

4.  Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation states, in pertinent part, that the total amount of foreign service completed during the period covered by the DD Form 214 will be entered in item 12f.  The regulation also states, in pertinent part, that the country and dates of deployment for an active duty Soldier deployed with his or her unit during their continuous period of active service will be shown in item 18 on the DD Form 214.

DISCUSSION AND CONCLUSIONS:

Although it appears the applicant was deployed in support of Operation Enduring Freedom and subsequently served in Iraq, his inclusive dates of service in Iraq are not available.  Therefore, there is insufficient evidence on which to amend item 18 on his DD Form 214 to show his dates of service in Iraq.
