      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 (Certificate of Release or
Discharge from Active Duty) be corrected by listing her foreign
service/deployments in support of Operation Enduring Freedom and Operation
Iraqi Freedom.

2.  The applicant states she was deployed to Cuba in support of Operation
Enduring Freedom, and to Tikrit, Iraq, in support of Operation Iraqi
Freedom.

3.  The applicant provides a copy of a certificate of achievement and a
recommendation for award of the Army Commendation Medal (ARCOM) for her
participation in Operation Enduring Freedom and Operation Iraqi Freedom.

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show she enlisted in the Regular Army on 12
June 2001.  After completion of basic and advanced individual training she
was awarded military occupational specialty 92Y (Unit Supply Specialist).
She was honorably discharged from active duty on 30 April 2005, after
completion of
3 years, 10 months and 19 days.  Her DD Form 214 indicates she had 1 year,
2 months and 7 days of Foreign Service.

2.  The applicant's records contain evidence that she was assigned to
Germany and was deployed to Iraq in support of Operation Iraqi Freedom,
from 1 October 2003 to 7 January 2004.

3.  The applicant's records contain a certificate of achievement for her
volunteer service with the Internal Revenue Service, as a tax assistor, in
Guantanamo Bay, Cuba, from 3 February 2003 to 19 March 2003.

4.  Evidence shows the applicant was awarded an Army Commendation Medal
(ARCOM) for meritorious service while deployed in support of Operation
Iraqi Freedom, from 1 October 2003 to 7 January 2004.

5.  The applicant provides a copy of a recommendation for award of the
ARCOM as well as an award certificate showing she was awarded an ARCOM for
meritorious service while deployed for Operation Iraqi Freedom, from 1
February 2000 to 1 February 2003, under Permanent Orders 344-001.



6.  Army Regulation 635-5 (Personnel Separations) established standardized
procedures for preparation and distribution of the DD Form 214.  The
regulation states for an active duty Soldier deployed with his or her unit
during their continuous period of active service enter in item 18 "Service
in (name of country deployed) and from (inclusive dates for example,
yyyymmdd - yyyymmdd)."

DISCUSSION AND CONCLUSIONS:

1.  Evidence of record shows that while the applicant was assigned to
Germany she was deployed to Iraq in support of Operation Iraqi Freedom from
1 October 2003 to 7 January 2004.  Therefore, her deployment to Iraq from 1
October 2003 to 7 January 2004, is authorized for inclusion in item 18
(Remarks) of her separation document.

2.  The applicant's records shows that while she was serving in Germany she
was transferred to Cuba from 3 February 2003 to 19 March 2003, as a
volunteer for the Internal Revenue Service, as an income tax assistor.
Documents recommending her for award of an ARCOM for the period 1 February
2000 to
1 February 2003, can not be validated.  The period covered by the award
begins in February 2000, however, she did not enter military service until
12 June 2001.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by adding the location and date of her deployment to
item 18 (Remarks) of her
DD Form 214 as "Service in Iraq from 20031001 – 20040107".




2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
the applicant being deployed to Cuba in support of Operation Enduring
Freedom, and her deployment to Iraq for the period 1 February 2001 to 1
February 2003.
