      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) for the period ending 26 March 2004 be
corrected to add the Global War on Terrorism Service Medal (GWOTSM) and the
Joint Service Achievement Medal (JSAM).

2.  The applicant states he was released from active duty due to a family
emergency and did not have time to fully out-process from Guantanamo Bay.

3.  The applicant provides his DD Form 214 for the period ending 26 March
2004; JSAM orders and citation; an SC Form 204 (USSOUTHCOM Recommendation
for Defense Award); two certificates of appreciation for the period
December 2003 through March 2004; and a Joint Interrogation Group, JTF –
GTMO (Joint Task Force – Guantanamo) certificate of service.

CONSIDERATION OF EVIDENCE:

1.  The applicant served in the Regular Army from 19 July 1984 through 18
July 1988, when he was released from active duty and transferred to the U.
S. Army Reserve.  He was ordered to active duty on 16 November 2003 in
support of Operation Enduring Freedom.  He was assigned to JTF Guantanamo,
Guantanamo Bay, Cuba in December 2003.

2.  JTF GTMO Permanent Order Number 58-18, dated 9 March 2004, awarded the
applicant the JSAM.

3.  On 26 March 2004, the applicant was released from active duty.  Item
      13 (Decorations, Medals, Badges, Citations and Campaign Ribbons
Awarded or Authorized) of his DD Form 214 for the period ending 26 March
2004 does not show the GWOTSM or the JSAM.

4.  Army Regulation 600-8-22 (Military Awards) states the GWOTSM is
authorized for award to members of the Armed Forces of the United States
who have participated in Global War on Terrorism operations outside of the
designated areas of eligibility on or after 11 September 2001 to a future
date      to be determined.  All Soldiers on active duty including Reserve
Component Soldiers mobilized or National Guard Soldiers activated on or
after 11 September 2001 to a date to be determined having served 30
consecutive days or              60 nonconsecutive days are authorized the
GWOTSM.

DISCUSSION AND CONCLUSIONS:

The applicant was awarded the JSAM, and he met the eligibility criteria for
award of the GWOTSM.  Therefore, his DD Form 214 for the period ending 26
March 2004 should be corrected to add these awards.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that the DD
Form 214 for the period ending 26 March 2004 of the individual concerned be
amended to add the Global War on Terrorism Service Medal and the Joint
Service Achievement Medal.
