	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of her latest Certificate of Release or Discharge from Active Duty (DD Form 214) to show her correct name and date of release from active duty; to add her military occupational specialty (MOS) of 33T1O; to add the Electronic Warfare/Intercept Tactical System Repair Course, the Wheel Vehicle Repairer course, the Nuclear Biological Chemical course, the Primary Leadership course, and the Medical Specialist course; to add the Global War on Terrorism Expeditionary Medal; and to correct the address of her nearest relative. 

2.  The applicant states that these errors occurred because she was not present to review the DD Form 214 and did not sign it. 

3.  The applicant provides copies of three DD Forms 214, separation orders and corrected copies, Request for  Early Release from Active Duty, Implementing message for Global War of Terrorism Medal, and e-mail dated 20 Jun 03. 

CONSIDERATION OF EVIDENCE:

1.  On 22 June 1982, the applicant enlisted in the United States Army Reserve (USAR).  She completed her active duty for training and was awarded MOS 91B1O (Medical Specialist).  Her DD Form 214 for this period of active duty shows in Item 11 (Primary Specialty Number) her primary MOS 91B1O.  Item 
14 (Military Education) of this form shows she completed the 6 week Medical Specialist Course in November 1982.

2.  On 1 February 1984, the applicant enlisted in the Regular Army.  She completed advanced individual training and was awarded MOS 33T1O (Electronic Warfare/Intercept Tactical systems Repairer).  

3.  On 1 April 1992, the applicant was honorably discharged.  She had completed 8 years, 2 months and 1 day of creditable active duty during this period.  Her DD Form 214 for this period of active duty shows in Item 11 (Primary Specialty) her primary MOS 33T1O.  Item 14 (Military Education) of this form shows she completed the Electronic Warfare/Intercept Tactical System Repairer Course, Wheel Vehicle Repairer Course, Nuclear Biological Chemical Course, and the Primary Leadership Development Course.

4.  On 7 February 1998, the applicant enlisted in the USAR for 6 years beginning in the rank of sergeant, pay grade E5.

5.  On 7 February 2003, the applicant was ordered to active duty in support of Operation Enduring Freedom. 

6.  E-mail communication between what appears to be the applicant’s unit personnel noncommissioned officer and another individual indicates that the applicant left Iraq on 7 June 2003 and arrived at Walter Reed Army Medical Center on 9 June 2003.  There is no available evidence showing when the applicant had arrived in the overseas theater.

7.  Letter Orders 034-57, Headquarters, Fort McCoy, Wisconsin, dated 
3 February 2004, released the applicant from active duty and transferred her back to her Reserve unit, effective 13 April 2004.  A “corrected copy” of this letter order was published changing the effective date to read “8 May 2004.

8.  The applicant’s DD Form 214 for the period of active duty commencing on
7 February 2003, shows in Item 1 (Name) F____, K____ P____ M____.  It shows in Item 11 (Primary Specialty) MOS 63W2O (Wheel Vehicle Repairer) and MOS 91W2O (Health Care Specialist).  It shows in Item 12b (Separation Date This Period) 13 April 2004.  It shows in Item 12c (Net Active Service This Period) 1 year, 2 months, and 7 days.  It indicates in Item 12f (Foreign Service) no foreign service during this period.  It does not show in Item 13 (Decorations, Medals, Badges, citations and Campaign Ribbons Awarded or Authorized (All Periods of Service)) the Global War on Terrorism Expeditionary Medal.  Item 14 (Military Education) shows None.   Item 19b (Nearest Relative) shows an address in Cambridge, Minnesota. 

9.  Records indicate that the applicant’s name is commonly referred to in her military records with her last name hyphenated, as in F____-K____, P____ M____, showing a coma between the second and third portion of her name.

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record clearly and convincingly shows that the applicant’s name was incorrectly entered on her latest DD Form 214.  Therefore, her name should be corrected.  

2.  The evidence clearly shows that the applicant’s letter orders initially contained the wrong date for her release from active duty and that it was subsequently corrected.  Consequently, it is reasonable to presume that this first error caused a similar error to be made on her DD Form 214.  Therefore, her DD Form 214 should be corrected to show her correct date of release as 8 May 2004.

3.  As a result of correcting the applicant's date of separation, Item 12c (Net Active Service This Period) should also be corrected to show 1 year, 3 months, and 2 days.

4.  The evidence of record clearly shows that all of the applicant’s military training courses are properly shown on her DD Forms 214.  The applicant is asking that all of her military training be consolidated onto her latest DD Form 214.  While the DD Form 214 provides for consolidating awards earned during all periods of service, it does not provide for consolidating other items such as training.  Therefore, her request to include all of her training onto her latest DD Form 214 should not be granted.

5.  The available evidence shows that the applicant held MOS 63W2O and 91W2O during her latest period of active duty. She has not provided any substantiating evidence showing that she still held MOS 33T2O. Therefore, her request to add this MOS to Item 11 should not be granted.

6.  The applicant’s DD Form 214 indicates that she was ordered to active duty in support of Operation Enduring Freedom.  However, there is insufficient evidence to determine from when to when or just how long she actually performed duty in the theater of operation.  Therefore, the applicant’s request for award of the Global War on Terrorism Expeditionary Medal should not be granted.  It is also  noted that Item 12f (Foreign Service) is devoid of her overseas service.  The available evidence is insufficient to determine how much overseas service the applicant has completed.  However, if the applicant has additional evidence that shows she served in the theater of operation for the requisite time, she may resubmit her application and include any supporting documentation.

7.  The applicant's DD Form 214 indicates that at the time of the applicant’s release from active duty, a determination was made as to who was her nearest relative and where that person lived.  The applicant requests that the address for this individual be changed.  She has not provided any evidence to show that the address was incorrect at the time of her separation.  Therefore, her request should not be granted.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned, and in particular, her DD Form 214 issued in 2004, be corrected by showing in:

	a.  Item 1 (Name): ”F____-K____, P____ M____; 

	b.  Item 12b (Separation Date This Period): 2004  05  08; and

	c.  Item 12c (Net Active Service This Period): 1 year, 3 months, 2 days.

2.  The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to the Global War on Terrorism Expeditionary Medal, military occupational specialty 33T2O, military training courses, and the address of her nearest relative.
