	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) with the period ending 5 April 2006 be corrected to show award of the National Defense Service Medal (NDSM).  He further requests that his DD Form 214 be corrected to show the correct date he entered active duty.

2.  The applicant states, in effect, that his DD Form 214 does not show he was awarded the NDSM.  He further states that the date he entered active duty on his DD Form 214 is not correct.

3.  The applicant provides his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s military records are not available to the Board.  This case is being considered using reconstructed records, which primarily consist of his DD Form 214 and DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States).  

2.  The applicant's DD Form 4 shows that he enlisted in the Regular Army on 
16 June 2004.  

3.  The applicant's DD Form 214 shows he was honorably discharged on 5 April 2006 by reason of disability after completing 1 year, 9 months, and 6 days of creditable active service.

4.  The applicant's DD Form 214 does not show award of the NDSM. 

5.  Army Regulation 600-8-22 (Military Awards) as amended provides that the National Defense Service Medal is awarded for honorable active service for any period between 27 July 1950 through 27 July 1954, 1 January 1961 through 14 August 1974, 2 August 1990 through 30 November 1995, and 11 September 2001 to a date to be determined.  

DISCUSSION AND CONCLUSIONS:

1.  Evidence of record shows that the applicant entered active duty on 16 June 2004.  His DD Form 214 should be corrected to show the correct active duty entry date.

2.  The applicant is eligible for award of the NDSM.  His DD Form 214 should be amended to add this medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  amending his DD Form 214 to add the award of the National Defense Service Medal; 

	b.  deleting the entry in item 12a "2006" year(s), "01" month(s), and "16" day(s) of his DD Form 214; and 
	
	c.  amending his DD Form 214 in item 12a to add "2004" year(s), "06" month(s), and "16" day(s).
