The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show award of the Combat Infantryman Badge (CIB).
2. The applicant states, in effect, when he was released from active duty, his CIB was not entered on his DD Form 214 because he did not have a copy of his orders.
3. The applicant provides copies of his DD Form 214 and orders awarding him the CIB.
CONSIDERATION OF EVIDENCE:
1. The applicant is a member of the U.S. Army Reserve. He was ordered to active duty in support of Operation Iraqi Freedom on 21 October 2004. He was honorably released from active duty on 20 January 2006.
2. Permanent Orders 093-025, Department of the Army, 155th Brigade Combat Team, FOB (Forward Operating Base) Kalsu, Iraq, dated 3 April 2005, awarded the applicant the CIB for the period 9 January 2005 through 16 February 2005.
DISCUSSION AND CONCLUSIONS:
1. The applicant served in Iraq from 9 January 2005 through 29 December 2005.
2. Permanent Orders awarded the applicant the CIB. Therefore, he is entitled to have his records corrected to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the applicant's DD Form 214 to show award of the CIB.
