      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Report of Separation from
Active Duty) for the period ending 12 July 1974 be corrected to show it was
signed by the authorizing officer.

2.  The applicant states that a clerical error was made by failing to have
the authorizing officer sign the DD Form 214.

3.  The applicant provides his DD Form 214 for the period ending 12 July
1974.

CONSIDERATION OF EVIDENCE:

1.  The applicant served in Operation Iraqi Freedom.

2.  The applicant enlisted in the Regular Army on 4 June 1968.  He was
honorably discharged on 14 July 1970 for the purpose of immediate
reenlistment.

3.  The applicant reenlisted on 15 July 1970 for 4 years.  He was honorably
discharged on 12 July 1974 in the rank and grade of Specialist Five, E-5
upon the expiration of his term of service.  He had completed a total of 6
years,           1 month, and 9 days of creditable active service.  His
copy of the DD Form 214 for the period ending 12 July 1974 shows the
authorizing officer did not sign the form.  The record copy of this DD Form
214 shows that neither the applicant nor the authorizing officer signed the
form.

4.  The two available copies of the applicant’s DD Form 214 for the period
ending 12 July 1974 also do not indicate the authority and reason for his
discharge (i.e., paragraph 2-18, Army Regulation 635-200; separation
designator code 201 expiration of term of service) or his reenlistment (RE)
code.

5.  The applicant enlisted in the U. S. Army Reserve on 24 August 1978 and
served in an active status until he was transferred to the Retired Reserve
on      11 November 2004 in the rank and grade of Chief Warrant Officer
Three.

6.  RE code 1 applies to persons fully qualified for continued Army
service.

DISCUSSION AND CONCLUSIONS:

1.  An error was made on the applicant’s DD Form 214 for the period ending
      12 July 1974 in that the authorizing officer failed to sign the
document.

2.  It would be appropriate to reissue the applicant’s DD Form 214 for the
period ending 12 July 1974, to include adding all missing information.  It
cannot be definitively determined at this point in time whether the
applicant would have received an RE code of 1 after his discharge of 12
July 1974.  However, as he subsequently enlisted in the U. S. Army Reserve,
later was appointed a warrant officer, and was recently transferred to the
Retired Reserve, it would be appropriate to show he did receive an RE code
of 1 at the time of his 12 July 1974 discharge.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
reissuing his DD Form 214 for the period ending 12 July 1974 to include
showing the authority and reason for his discharge as paragraph 2-18, Army
Regulation 635-200 with a separation designator code 201 (expiration of
term of service) and showing his RE code as RE code 1.
