                            RECORD OF PROCEEDINGS

      IN THE CASE OF:

      BOARD DATE: 27 February 2007
      DOCKET NUMBER: AR20060009973

I certify that hereinafter is recorded the true and complete record
of the proceedings of the Army Board for Correction of Military Records in
the case of the above-named individual.

| |Mr. Gerard W. Schwartz | | Acting Director |
| |Mr. Joseph A. Adriance | |Analyst |

The following members, a quorum, were present:

| |Mr. Ted S. Kanamine | |Chairperson |
| |Mr. Larry C. Bergquist | |Member |
| |Mr. LaVerne M. Douglas | |Member |

The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, that his 18 June 2004 separation
document (DD Form 214) be corrected to show he served in military
occupational specialty (MOS) 88H (Cargo Specialist) in Item 11 (Primary
Specialty), and that his service in Iraq be documented in Item 18
(Remarks).

2. The applicant states, in effect, that he served in MOS 88H and not in
MOS 63S (Heavy Wheeled Vehicle Mechanic) as is currently shown in Item 11
of his DD Form 214. He also indicates that Item 18 (Remarks) fails to
document his service in Iraq in March and April 2003.

3. The applicant provides the following documents in support of his
application: DD Form 214; Certificate of Achievement; United States Army
Transportation School Diploma, dated 10 October 2002; and Recommendation
for Award (DA Form 638), dated 17 March 2004.

CONSIDERATION OF EVIDENCE:

1. The applicant's record shows that he enlisted in the Regular Army and
entered active duty on 23 May 2002. His Enlisted Records Brief (ERB) and
Personnel Qualification Record (PQR), DA Form 2-1, show that he completed
basic combat training at Fort Jackson, South Carolina, and advanced
individual training (AIT) at Fort Eustis, Virginia.

2. On 10 October 2002, upon his successful completion of AIT, the
applicant was awarded MOS 88H (Cargo Specialist), and was assigned to 551st
Transportation Company, Fort Eustis.

3. The applicant's record contains a DA Form 638 that shows he was awarded
the Army Achievement Medal (AAM) for his meritorious service with the 551st
Transportation Company during the period 2 November 2002 through 1 June
2004. The list of achievements contained on this recommendation includes
his service while deployed in support of Operation Enduring Freedom and
Operation Iraqi Freedom.

4. On 18 June 2004, the applicant was honorably retired by reason of
disability after completing 2 years and 26 days of active military service.
 Item 11 of the
DD Form 214 he was issued contains an entry showing he served in MOS 63S10
(Heavy Wheeled Vehicle Mechanic) for 1 year and 8 months.

5. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons
Awarded or Authorized) shows he earned the following awards during his
active duty tenure: AAM; National Defense Service Medal; Army Service
Ribbon; Global War on Terrorism Expeditionary Medal; and Global War on
Terrorism Service Medal. Item 18 contains an entry that indicates the
applicant served in Kuwait from 8 February through 28 April 2003.

6. The applicant provides a Certificate of Achievement he received for his
outstanding patriotism during Operation Iraqi Freedom. This certificate
was signed by his battalion commander at Camp Cedar, Iraq.

7. Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that must be prepared for Soldiers on retirement, discharge,
release from active duty service, or control of the Active Army. It also
establishes standardized policy for preparing and distributing the DD Form
214. Chapter 2 contains instructions for completing the DD Form 214. The
instructions for Item 11 state to enter the titles of all MOSs served for
at least 1 year and include for each MOS the number of years and months
served, as documented on the ERB. The instructions also state, in pertinent
part, that for an active duty Soldier deployed with his or her unit during
the period covered by the DD Form 214, an entry will be made showing the
name of the country to which deployed and the period of service in that
country in Item 18.

DISCUSSION AND CONCLUSIONS:

1. The applicant's contention that his separation document should be
corrected to reflect his actual MOS and to document his Iraq service was
carefully considered and found to have merit.

2. The applicant's ERB and PQR confirm he was trained in and awarded MOS
88H on 10 October 2002, and that he served in this MOS for 1 year and
8 months until his separation on 18 June 2004. Therefore, it would be
appropriate to amend Item 11 of his DD Form 214 by deleting the current
entry and replacing it with an entry that reflects this service in MOS 88H
as a Cargo Specialist.

3. The applicant's record also confirms that during his deployment with
his unit in support of Operation Enduring Freedom and Operation Iraqi
Freedom, he served in both Kuwait and Iraq; however, the break down of the
periods served in each of these countries is not available. Therefore, it
would be appropriate to amend the pertinent entry in Item 18 to reflect
service in Kuwait and Iraq during the period 8 February through 28 April
2003.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending his 18 June 2004 DD Form 214 as follows:

      a. delete the current entry in Item 11 and replace it with the entry

"88H10 00 --  Cargo Specialist --  1 Year-8 Months;

      b. deleting the current portion of the Item 18 entry pertaining to
deployed service in Kuwait and replacing it with the entry "SERVICE IN
KUWAIT AND IRAQ FROM 8 FEBRUARY 2003 TO 28 APRIL 2003; and

      c. providing him a correction to his separation document that
reflects these changes.
