THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of item 7b (Home of Record) on his
DD Form 214 (Certificate of Release or Discharge from Active Duty) to show
his Home of Record (HOR) as Gresham, Oregon.
2. The applicant states it was pointed out to him around August 2005 that
his HOR was indicated as Prescott, Arizona. He states that he immediately
attempted to have this information corrected. He states he has intended to
have his HOR designated as Gresham, Oregon as indicated on his officer
accessions packet. His DA Form Letter 1 indicated his address as Gresham,
Oregon and he has always maintained his residence in Gresham, Oregon. In
2002, he submitted a Pay Inquiry to correct his state tax information from
Arizona to Oregon and to have his Officer Record Brief corrected to reflect
Gresham, Oregon as his HOR. He was not aware or informed that any further
action needed to be taken at that time.
3. The applicant provides his separation orders; his letter to the Branch
Manager at Fort Bliss, Texas indicating his correct HOR; his DD Form 214;
his Officer Record Brief; his state of Oregon driver’s license; his
appointment letter; a Reserve Officer’s Training Corps (ROTC) Cadet Branch
Form; and his Assignment Preference Sheet.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the U.S. Army Reserve on 3 September 1998 as
a cadet. His DD Form 4/1 (Enlistment/Reenlistment Document) shows his HOR
as Gresham, Oregon. His records contain a DA Form 597-3 (Army Senior ROTC
Scholarship Cadet Contract) which shows his HOR as Gresham, Oregon. He
attended ROTC at Northern Arizona University.
2. The applicant was issued an Oregon driver’s license on 24 August 1999.
3. The applicant underwent a physical examination on 9 June 2000. His
Report of Medical Examination shows his home address as Gresham, Oregon.
4. The applicant completed an Assignment Preference Sheet on 28 August
2001 which shows his permanent home address as Gresham, Oregon.
5. He was appointed as a Reserve commissioned officer on 15 December 2001.
His letter of appointment shows his home address as Gresham, Oregon.
However, his active duty orders show his HOR as Prescott, Arizona.
6. Section IV (Personal/Family Data) on the applicant’s Officer Record
Brief, dated 4 October 2005, shows his HOR at his entry on active duty
(EAD) as “OREGON.”
7. Headquarters, U.S. Army Air Defense Artillery Center and Fort Bliss
Orders Number 252-0023, dated 9 September 2005, released the applicant from
active duty on 14 December 2005 and transferred him to the U.S. Army
Reserve Control Group (Reinforcement) on the following date. These orders
show his HOR as “PRESCOTT, AZ.”
8. Headquarters, U.S. Army Air Defense Artillery Center and Fort Bliss
Orders Number 005-005 were published on 5 January 2006 which amended Orders
Number 252-0023, dated 9 September 2005, to change the applicant’s HOR to
show “GRESHAM, OR.”
9. Item 7b on the applicant’s DD Form 214 shows his HOR as “PRESCOTT
ARIZONA.”
10. Army Regulation 635-5 prescribes the separation documents prepared for
Soldiers upon retirement, discharge, or release from active military
service or control of the Army. It establishes standardized policy for the
preparation of the DD Form 214. It provides a brief, clear-cut record of
active Army service at the time of release from active duty, retirement or
discharge. In pertinent part, it directs that the HOR at time of entry
will be entered in Block 7b of the DD Form 214. This regulation states
that the street, city, state and zip code will be entered. HOR is the
place recorded as the home of the individual when commissioned, appointed,
enlisted, inducted, or ordered into the relevant tour of active duty. The
place recorded as the home of the individual when reinstated, reappointed,
or reenlisted remains the same as that recorded when commissioned,
appointed, enlisted, or inducted or ordered into the relevant tour of
active duty unless there is a break in service of more than one full day.
DISCUSSION AND CONCLUSIONS:
1. The applicant enlisted in the U.S. Army Reserve on 3 September 1998.
At that time, the city and state of his HOR were recorded as Gresham,
Oregon. However, his DD Form 214 incorrectly shows the state of his HOR as
Prescott, Arizona in item 7b. All his pre-service documents except his
active duty orders show his HOR as Gresham, Oregon. It appears that,
because he was attending Northern Arizona University at the time, his
active duty orders erroneously showed his HOR as Prescott, Arizona.
2. It appears that an administrative error has occurred in this case
regarding the applicant's HOR. It would be appropriate to correct item 7b
on the applicant's DD Form 214 to show his HOR as Gresham, Oregon.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending item 7b on his DD Form 214 to show the HOR as Gresham, Oregon.
