The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Armed Forces of the United
States Report of Transfer or Discharge) be corrected to show he separated
in the pay grade of E-4.
2. The applicant states that he was promoted to pay grade E-4 on 1 October
1967 during his tour in Vietnam.
3. The applicant provides copies of his promotion orders, his Installation
Clearance Record (DA Form 137), and a letter from the Department of the
Army, Office of the Adjutant General, concerning a change in his military
occupational specialty (MOS) while assigned to annual training, in support
of his request.
CONSIDERATION OF EVIDENCE:
1. The applicant was inducted into the Army of the United States on 2 May
1966 for a period of 2 years. He served in Vietnam from January 1967 to
January 1968.
2. Special Order Number 274, Headquarters, 9th Infantry Division, APO San
Francisco, dated 1 October 1967, promoted the applicant to Specialist Four,
pay grade E-4 on 1 October 1967.
3. General Order Number 980, Department of the Army, Headquarters, 9th
Infantry Division, APO San Francisco, dated 25 February 1968, awarded the
applicant the Army Commendation Medal for action in Vietnam from 26 August
1967 to 9 January 1968, the grade of Specialist Four.
4. On 16 April 1968, the applicant’s Installation Clearance Record shows
the applicant’s grade as Specialist Four (SP4) and shows his conduct and
efficiency rated as excellent.
5. On 17 April 1968, the applicant’s Medical Examination and Mental Status
Evaluation cleared the applicant for separation and showed his grade as
SP4.
6. Several letters from the Department of the Army, Office of the Adjutant
General, concerning his physical condition, and his Reserve status
addressed the applicant as SP4.
7. The applicant’s discharge orders from the United States Army Reserve,
dated 19 April 1972, show the applicant’s grade as SP4.
8. The applicant’s DD Form 214, shows the applicant was honorably
discharged upon expiration term of service on 1 May 1968, in the pay grade
of E-3, and transferred to the United States Army Reserve. The applicant
had 2 years of creditable service.
DISCUSSION AND CONCLUSIONS:
1. Evidence adequately shows the applicant was promoted to pay grade E-4
on 1 October 1967.
2. The applicant’s Installation Clearance Records shows his pay grade as
Specialist Four. His conduct and efficiency ratings were shown as
excellent, which tends to show that he was in good standing and in no
jeopardy of having promotion revoked.
3. The applicant’s DD Form 214 should be corrected to show he was
discharged in the grade of Specialist Four, pay grade E-4, with a date of
rank of 1 October 1967.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
changing his DD Form 214, to show he was discharge in the grade of
Specialist Four, pay grade E-4, with a date of rank of 1 October 1967.
