The Board considered the following evidence:

if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that his DD Form 4/1 (Enlistment
/Reenlistment Document), Section A, Item 6 (Date of Birth) and his DD Form
214 (Certificate of Release or Discharge from Active Duty), Item 5 (Date of
Birth) be corrected from 4 November 1981 to 4 November 1980.
2. The applicant states, in effect, that his enlistment contract and DD
Form 214 erroneously shows his date of birth as 4 November 1981.
3. In support of his application, the applicant submits a Memorandum for
Record (MFR), Subject: Request DD Form 215 (Correction to DD Form 214) and
Request Correction to DD Form 4/1, from the Headquarters, United States
Army Recruiting Command (USAREC) Liaison, Fort Leonard Wood, Missouri,
dated 27 June 2005; a copy of his DD Form 4/1, Enlistment /Reenlistment
Document, page 1, dated 23 June 2005; his DD Form 214, dated 19 April 2005;
his Regular Army Applicant Data Report; a copy of pertinent pages of his
passport; and his Statement of Understanding, dated 27 June 2005.
CONSIDERATION OF EVIDENCE:
1. DD Form 4/1, Section A, Item 6 and DD Form 214, Item 5 shows that the
applicant's date of birth as 4 November 1981.
2. The applicant's passport shows his date of birth as 4 November 1980.
3. The MFR from USAREC dated 27 June 2005, confirms that the DD Form 4/1
and DD Form 214 are incorrect and the applicant's date of birth should be
corrected to reflect his date of birth as 4 November 1980.
4. The applicant signed a Statement of Understanding to acknowledge that
he understood that his request to amend his enlistment contract and DD Form
214 was for his benefit and was not submitted under coercion.
DISCUSSION AND CONCLUSIONS:
1. An error was made during the preparation of the applicant's enlistment
contract and DD Form 214, which resulted in an incorrect date of birth
being shown on his DD Form 4/1 and DD Form 214.
2. The applicant's DD Form 4/1, Section A, Item 6 and DD Form 214, Item 5,
erroneously show his date of birth as 4 November 1981. The MFR from
USAREC dated 27 June 2005, confirms that the DD Form 4/1 and DD Form 214
are incorrect and should be corrected to reflect his date of birth as
4 November 1980.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending the DD Form 4/1 and DD Form 214, of the individual concerned, to
show his date of birth as 4 November 1980 instead of 4 November 1981 that
is currently shown.
