The Board considered the following evidence:

if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests award of the Good Conduct Medal (5th award).
2. The applicant states that while assigned to an active component, the
original request for the 5th award of the Good Conduct Medal was never
awarded or got lost. He continues that a second request for the medal was
made in 2004 and still no action.
3. The applicant provides a copy of DA Form 4187 (Personnel Action), dated
26 March 2004; a copy of 402nd Quartermaster Battalion memorandum, dated
2 March 2004; a copy of his DD Form 214 (Certificate of Release or
Discharge from Active Duty) with a separation date of 8 February 2003; and
a copy of 55th Personnel Services Battalion Order 074-022, dated 15 March
1999 in support of his application.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Army on 11 March 1987 for a period of 3
years. After completion of basic and advanced individual training, he was
awarded military occupational specialty 77F10 (Petroleum Supply
Specialist). The applicant continued to serve through a series of
reenlistments. On 6 January 1999, he reenlisted for an indefinite period
in MOS 77F30 (Petroleum Supply Specialist) and was assigned to the 99th
Regional Support Command. On 8 February 2003, the applicant was honorably
discharged.
2. The applicant's DD Form 214 does not show award of the Good Conduct
Medal (5th Award). The applicant's service records do not contain any
orders awarding him the Good Conduct Medal.
3. Records show the applicant received award of the Good Conduct Medal
(4th Award) for the period of service 11 March 1996 through 10 March 1999.
4. A review of the applicant's service records for the period of active
duty service from 11 March 1999 through 10 March 2002 shows no derogatory
information that would disqualify him for award of the Good Conduct Medal
(5th Award).
5. The applicant submitted a copy of 402nd Quartermaster Battalion
memorandum, dated 2 March 2004 to the Military Personnel Division, Fort
McPherson, Georgia. This memorandum states the applicant was an active
component Soldier and a request for award of the Good Conduct Medal (5th
Award) was submitted in June 2002 for the period of service 11 March 1999
through 10 March 2002. The memorandum also indicates the applicant is
requesting a copy of the orders awarding him the Good Conduct Medal (5th
Award).
6. The applicant submitted a copy of DA Form 4187, dated 26 March 2004,
which shows the applicant's unit commander recommended approval of the
applicant's request for award of the Good Conduct Medal (5th award).
7. Army Regulation 600-8-22 provides that the Good Conduct Medal is
awarded to individuals who distinguish themselves by their conduct,
efficiency and fidelity during a qualifying period of active duty enlisted
service. This period is 3 years except in those cases when the period for
the first award ends with the termination of a period of Federal military
service. Although there is no automatic entitlement to the Good Conduct
Medal, disqualification must be justified.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends he is entitled to award of the Good Conduct
Medal (5th Award) for his period of active service from 11 March 1999
through 10 March 2002.
2. Evidence of record confirms that the applicant served honorably during
the period from 11 March 1999 through 10 March 2002. Lacking any
derogatory information on file that would have disqualified him, it would
be appropriate to award the applicant the Good Conduct Medal for the 3 year
qualifying period from 11 March 1999 through 10 March 2002. Therefore, he
is entitled to correction of his records to show award of the Good Conduct
Medal (5th Award) for this period of service.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
awarding the Good Conduct Medal (5th Award) for the 3 year qualifying
period from 11 March 1999 through 10 March 2002.
