      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) should be corrected to show he served in
Afghanistan.

2.  The applicant states he was ordered to active duty on 10 May 2004 and
assigned to the United States Army Central Command Bagram (WATGD2)
Afghanistan and returned to the United States on 13 September 2004.

3.  The applicant provides a copy of his DD Form 214, 88th Regional
Readiness Command Orders 04-022-00113, Headquarters 88th Regional Support
Command Orders 04-030-00118, Headquarters Fort McCoy Permanent Orders 131-
01, Annex A to Permanent Orders 131-01, AF Form 3899 (Aeromedical
Evacuation Patient Record), Headquarters 1st Personnel Command Orders 265-
010, U.S. Army Human Resources Command Orders A-09-409966, Headquarters
United States Army Armor Center and Fort Knox Orders 338-0377, and
Headquarters Fort McCoy Order 018-30.

CONSIDERATION OF EVIDENCE:

1.  Records show the applicant enlisted in the Army Reserve on 30 March
1976 for a period of 6 years.  The applicant continued to serve in the Army
Reserve through a series of reenlistments.  On 11 February 2004, while
assigned to 367th Engineer Battalion in Fort McCoy, Wisconsin, the
applicant was ordered to active duty.  He was honorably released from
active duty on 14 January 2005 for completion of required active service.

2.  Item 12f (Foreign Service) of the applicant's DD Form 214, with a
separation date of 14 January 2004, contains the entry "00 years, 00
months, 00 days."  Item 18 (Remarks) of this form contains the following
entries "Ordered to Active Duty in Support of Operation Enduring Freedom
IAW 10 USC 12302//Member has Completed First Full Term of Service."

3.  The applicant's DD Form 214 does not reflect any foreign service.

4.  88th Regional Readiness Command Orders 04-022-00113, dated 22 January
2004, ordered the applicant's unit, 367th Engineer Battalion, to active
duty for the purpose of mobilization for Enduring Freedom (Other Than
Homeland) effective 11 February 2004.

5.  Headquarters Fort McCoy Permanent Orders 131-01, dated 10 May 2004,
assigned the applicant's unit to Headquarters, United States Army Central
Command Bagram (WATGD2) Bagram, Afghanistan, effective 12 May 2004.

6.  Annex A to Permanent Orders 131-01, dated 10 May 2004, pertaining to
the temporary change of station to Afghanistan, lists the applicant's name.

7.  AF Form 3899 (Aeromedical Evacuation Patient Record), dated
10 September 2004, shows the applicant was admitted to Bagram Army Hospital
and diagnosed with a Deep Vein Thrombosis [Definition: the formation or
presence of a thrombus (a clot of coagulated blood attached at the site of
its formation) in a blood vessel].  This form further shows the applicant
needed long term therapy; however, it could not be done in the theater.

8.  Headquarters, 1st Personnel Command Orders 265-010, dated 21 September
2004, reassigned the applicant to Headquarters and Headquarters Company,
367th Engineer Battalion, Fort McCoy, Wisconsin, effective 21 September
2004, for purpose of further medical care.

9.  U.S. Army Human Resources Command Orders A-09-409966, dated
30 September 2004, retained the applicant on active duty for the purpose of
voluntarily participation in the Reserve Component Medical Holdover Medical
Retention Processing Program for completion of medical care and treatment.
These orders indicated that the applicant is to report to the Medical
Retention at Fort Knox, Kentucky on 30 September 2004 for a period of 179
days.

10.  Army Regulation 635-5 (Separation Documents) establishes the
standardized policy for preparing and distributing the DD Form 214.  The
regulation in effect at the time directs, in pertinent part, that the
purpose of the separation document is to provide the individual with
documentary evidence of their military service.  It is important that
information entered on the form should be complete and accurate.

11.  Paragraph 2-4 of Army Regulation 635-5 indicates that item 12f
(Foreign Service) of the DD Form 214 would show the total amount of foreign
service completed during the period covered in block 12c (Net Active
Service This Period), which will be taken from the Soldier's Enlisted
Record Brief or Officer Record Brief.



12.  Paragraph 2-4 of Army Regulation 635-5 also indicates that a Reserve
Soldier ordered to active duty and deployed to a foreign country, the
following three statements, in succession, will be entered in item 18
(Remarks) of the DD Form 214:

      1) “ORDERED TO ACTIVE DUTY IN SUPPORT OF (OPERATION NAME) PER 10 USC
(applicable section).”


      2) “SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for
example, YYYYMMDD -YYYYMMDD).”

      3) “SOLDIER COMPLETED PERIOD FOR WHICH ORDERED TO ACTIVE DUTY FOR
PURPOSE OF POSTSERVICE BENEFITS AND ENTITLEMENTS.”

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should be corrected to show
his service in Afghanistan.

2.  Evidence clearly shows the applicant was ordered to active duty in
support of Operation Enduring Freedom and was deployed to Afghanistan for
the period 12 May 2004 through 21 September 2004.  Therefore, the applicant
is entitled to correction of his DD Form 214 to show this Foreign Service.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned by:

      a) correcting item 12f of the DD Form 214 to show his total foreign
service as "00 years, 04 months, 09 days"; and


      b) by adding the following entry in item 18 of the DD Form 214,
"SERVICE IN AFGHANISTAN FROM 20040512–20040921."
