      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his 22 May 2004 DD Form 214 (Certificate of
Release or Discharge from Active Duty) be corrected to show four awards of
the Army Commendation Medal, three awards of the Army Achievement Medal,
and award of the Armed Forces Service Medal.

2.  The applicant states that all the information on his 31 August 1996
         DD Form 214 was not carried forward to his 22 May 2004 DD Form
214.

3.  The applicant provides a copy of the above-mentioned DD Forms 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant was appointed a Medical Corps officer in the Army Reserve
on 7 June 1984.  He served on active duty from 9 July 1987 to 8 July 1991.
His    DD Form 214 shows award of the Joint Service Achievement Medal,
National Defense Service Medal, Army Service Ribbon, and Army Commendation
Medal.
2.  The applicant’s Official Military Personnel File (OMPF) shows that he
was awarded the Army Commendation Medal on two occasions, for the period 9
July 1987 to 8 July 1991; and for the period 22 January 1994 to 8 February
1994.  There is no evidence contained in his OMPF of more than two awards
of the Army Commendation Medal.

3.  The applicant’s OMPF shows that he was awarded the Army Achievement
Medal on two occasions, for the period 27 March 1994 to 8 April 1994; and
for the period 2 June 1999 to 18 June 1999.  There is no evidence contained
in his OMPF of more than two awards of the Army Achievement Medal.

4.  The applicant’s 31 August 1996 DD Form 214 shows that he was ordered to
active duty in support of Operation Joint Endeavor, and that he served in
Germany from 7 May 1996 to 25 August 1996.  His DD Form 214 shows award of
the Army Commendation Medal (2nd award), award of the Army Achievement
Medal, and award of the Armed Forces Service Medal.

5.  The applicant’s 22 May 2004 DD Form 214 shows that he was ordered to
active duty in support of Operation Enduring Freedom and that he served in
Kosovo from 29 October 2003 to 26 April 2004.  His DD Form 214 shows award
of the Army Commendation Medal (3rd award), Army Achievement Medal (2nd
award), and Global War on Terrorism Service Medal.  Award of the Armed
Forces Service Medal is not reflected on this DD Form 214.
DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded the Armed Forces Service Medal as shown on
his 31 August 1996 DD Form 214.  His 22 May 2004 DD Form 214 should be
corrected to show that award.

2.  The applicant’s 22 May 2004 DD Form 214 shows that he was awarded the
3rd award of the Army Commendation Medal and the 2nd award of the Army
Achievement Medal.  There is no evidence, and the applicant has not
submitted any, to show that he was awarded the 4th award of the Army
Commendation Medal or the 3rd award of the Army Achievement Medal.
Consequently, his request in this respect cannot be granted.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that the applicant’s 22 May 2004 DD Form 214 be corrected by
showing award of the Armed Forces Service Medal.

2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
a 4th award of the Army Commendation Medal and a 3rd award of the Army
Achievement Medal.
