The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of
Release or Discharge from Active Duty) for the period ending 31 October
1994 to show her specialty as 72C (Audiologist).
2. The applicant states she has been an audiologist and served in
specialty 72C since she was commissioned in 1983. She states that her
retirement points do not currently reflect her active duty service from
1988 to 1994 because of this error.
3. The applicant provides her DD Form 214 Worksheet for the period ending
30 July 2004; her DD Form 214 for the period ending 31 October 1994; her
DA Form 2B (Personnel Qualification Record); her DA Form 2-1 (Personnel
Qualification Record- Part II); her DA Form 67-8 (Officer Evaluation
Report);
and her Reserve retirement points.
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed a second lieutenant in the Medical Service
Corps on 13 June 1983 in the U.S. Army Reserve (USAR). Her appointment
letter shows her specialty as 68M (Audiologist). At the time the applicant
submitted her application, she was serving in the Reserve in the rank of
lieutenant colonel.
2. She was ordered to active duty on 14 August 1988 in specialty 68M.
3. The applicant's Personnel Qualification Record shows she served in duty
specialty 68M from September 1983 through November 1993.
4. The applicant's Officer Evaluation Reports for the periods covering 14
August 1988 through 4 November 1993 show her specialty as 68M and her
principal duty position as Chief of Audiology Service.
5. The applicant's Officer Evaluation Report for the period covering 5
November 1993 through 21 October 1994 shows her specialty as 72C and her
principal duty position as Chief of Audiology Service.
6. The applicant was honorably released from active duty on 31 October
1994. She completed 6 years, 2 months, and 17 days of active military
service during the period covered by the report. She was transferred to
the U.S. Army Reserve Control Group (Reinforcement) on 1 November 1994.
7. Item 11 (Primary Specialty) on the applicant's DD Form 214 for the
period ending 31 October 1994 shows the entry "67C9C HEALTH SERVICES
COMPTROLLER – 6-YEARS – 2 MOS//NOTHING FOLLOWS."
8. The applicant was ordered to active duty on 17 January 2003 in support
of Operation Enduring Freedom. She was released from active duty on 30
July 2004 and transferred back to her Reserve unit in San Antonio, Texas.
Her DD Form 214 for the period ending 30 July 2004 shows her primary
specialty as 72C.
9. The applicant's Officer Evaluation Report for the period covering 7
August 2003 through 30 July 2004 shows her specialty as 72C and principal
duty position as a Staff Audiologist.
10. Army Regulation 635-5 (Separation Documents) governs the preparation
of the DD Form 214. It states that the DD Form 214 is a synopsis of the
Soldier's most recent period of continuous active duty. It provides a
brief, clear-cut record of active Army service at the time of release from
active duty, retirement or discharge. In pertinent part, it directs that
the titles of all areas of concentration (AOC) served for at least one year
and the number of years and months served will be entered in item 11. For
commissioned officers, the AOC consists of two numerical digits and one
letter.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record shows the applicant was appointed in the Medical
Service Corps on 13 June 1983 in specialty 68M as an audiologist.
2. The applicant was ordered to active duty in August 1988 in specialty
68M.
3. The evidence of record shows the applicant held specialty 68M until
November 1993. It appears that specialty 68M was converted to specialty
72C around 1994.
4. The applicant continued to serve in specialty 72C until she was
released from active duty on 31 October 1994. However, her DD Form 214 for
the period ending 31 October 1994 shows her primary specialty as 67C9C.
5. It appears that an administrative error has occurred in this case
regarding the applicant's specialty. Therefore, it would appropriate to
amend item 11 on her DD Form 214 for the period ending 31 October 1994 to
show the entry "72C AUDIOLOGIST – 6 YEARS – 2 MONTHS."

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected:
a. by deleting the entry "67C9C HEALTH SERVICES COMPTROLLER – 6 YRS –
2 MOS" from item 11 on her DD Form 214 for the period ending 31 October
1994; and
b. by adding the entry "72C AUDIOLOGIST – 6 YEARS – 2 MONTHS" to item
11 on her DD Form 214 for the period ending 31 October 1994.
