      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or
Discharge From Active Duty) for the period 2003021-20040209 be corrected to
show his grade as Staff Sergeant (SSG/pay grade E-6) and that he received a
third award of the Army Commendation Medal.

2.  The applicant states that he was mobilized for 1 year during which time
he was promoted and awarded a third Army Commendation Medal.  He did not
have orders for his promotion and award when he was separated, so the
demobilization center did not make the correct entries at the time of his
separation.

3.  The applicant provides:

      a.  A self-authored memorandum dated 7 February 2004.

      b.  A 7 February 2004 memorandum from his US Army Reserve (USAR)
officer-in-charge.

      c.  Orders 03-079-00005, Headquarters, 143rd TRANSCOM, Orlando,
Florida, dated 20 March 2003, promoting him to SSG with a promotion date of
20 March 2003.

      d.  A copy of an Army Commendation Medal Certificate citing Permanent
Orders #206-162, 377th Theater Support Command, APO AE 09366, awarding the
Army Commendation Medal to the applicant for service from 19 March 2003 to
15 June 2003.  The Certificate is dated 25 July 2003.

      e.  A copy of the subject DD Form 214.

      f.  A copy of the applicant's DA Form 2166-8 (NCO Evaluation Report)
for the period 200207 to 200306 showing his rank as SSG.

      g.  A copy of a memorandum dated 29 September 2003 showing the
applicant, a SSG, was eligible to reenlist.

      h.  A copy of Orders 030-0035, Headquarters, 3rd Infantry Division
and Fort Stewart, Fort Stewart, Georgia, dated 30 January 2004, separating
the applicant as a SSG.

      i.  A copy of a 4 August 2003 memorandum from Headquarters, 143rd
TRANSCOM, APO AE 09366, requesting the applicant's release from active
duty.  His rank is shown as SSG.

      j.  A copy of Orders 66-97, Headquarters, Fort Stewart, Georgia,
dated 7 March 2003, deploying the applicant to Kuwait.  His grade is shown
as Sergeant (SGT/pay grade E-5).

      k.  A copy of Orders M-037-0042, Headquarters, 143rd TRANSCOM, dated
6 February 2003, ordering the applicant to active duty.  His grade is shown
as Sergeant (SGT/pay grade E-5).

CONSIDERATION OF EVIDENCE:

1.  The applicant is a USAR soldier who was ordered to active duty for 1
year on 10 February 2003.  His grade at the time was SGT.

2.  On 20 March 2003, the applicant was promoted to the grade of SSG.  He
was serving in Kuwait at the time.

3. On 25 July 2003, the Commanding General, 377th Theater Support Command
awarded the applicant the Army Commendation Medal.

4.  The applicant's Personnel Electronic Records Management System (PERMS)
fiche shows two previous awards of the Army Commendation Medal.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's DD Form 214 shows in Item 4a (Grade, Rate, or Rank)
that he is a SGT; it shows in Item 4b (Pay Grade) that he is an E-5; and it
shows in Item 13 (Decorations, etc.) that he was awarded the Army
Commendation Medal (2nd Award).

2.  The applicant has provided proof that he is a SSG/pay grade E-6 and
that he was awarded a third Army Commendation Medal.

3.  The applicant's DD Form 214 should be corrected to show the appropriate
rank, pay grade, and number of Army Commendation Medals awarded.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
issuing to him a DD Form 215 (Correction to DD Form 214) showing his rank
as SSG, his pay grade as E-6, and that he was awarded the Army Commendation
Medal (3rd Award).
