THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of item 12a (Date entered AD [Active
Duty] This Period) on her DD Form 214 (Certificate of Release or Discharge
from Active Duty) to show 13 April 1983; correction of item 12c (Net Active
Service This Period) to show 20 years and 18 days; and correction of item
12d (Total Prior Active Service) to show the entry, "0000 00 00."
2. The applicant states that the dates in these items are incorrect on her
DD Form 214. She also contends that these errors are affecting her
Department of Veterans Affairs disability rating.
3. The applicant provides a copy of her DD Form 214 and retirement
documentation.
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed a second lieutenant in the U.S. Army
Reserve on 1 April 1983 and ordered to active duty on 13 April 1983. She
was appointed a major in the Regular Army on 16 December 1993 and retired
in the rank of major on 30 April 2003.
2. Item 12a on the applicant's DD Form 214 shows she entered active duty
on
16 December 1993. Item 12c on her DD Form 214 shows she completed
9 years, 4 months, and 15 days. Item 12d on her DD Form 214 shows she
completed 10 years, 8 months, and 3 days of prior active service. Item 12e
(Total Prior Inactive Service) on her DD Form 214 shows that she completed
12 days. Item 18 (Remarks) on the applicant's DD Form 214 shows the entry,
"PERIOD OF DELAYED ENTRY PROGRAM: 19830401-19830417."
3. The applicant provided a DA Form 7301-R (Officer Service Computation
for Retirement) which shows she was in the delayed entry program from 1
April 1983 through 12 April 1983, that she entered active duty on 13 April
1983, and that she completed 20 years and 18 days of creditable active
service.
4. Retirement orders, dated 20 February 2003, show the applicant retired
on
30 April 2003 after completing 20 years and 18 days of creditable active
service.
5. Army Regulation 635-5 (Separation Documents) states that a DD Form 214
will not be prepared for soldiers on active duty terminating their Reserve
Component status to integrate in the Regular Army.
DISCUSSION AND CONCLUSIONS:
1. Records show the applicant was in the delayed entry program for 12
days, from 1 April 1983 through 12 April 1983. Records also show that she
entered active duty on 13 April 1983 and retired on 30 April 2003 after
completing 20 years and 18 days of continuous creditable active service.
2. By regulation, the applicant should not have been issued a DD Form 214
when she integrated into the Regular Army on 16 December 1993 and it
appears that she was not issued one. Therefore, she had continuous active
duty from 13 April 1983, not 16 December 1993. It would be appropriate
to correct item 12a on her DD Form 214 to show the applicant entered active
duty on 13 April 1983; to correct item 12c to show that she completed 20
years and 18 days; to correct item 12d to show the entry, "0000 00 00;" and
to correct item 18 to show that she was in the delayed entry program from 1
April 1983 through 12 April 1983.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by:
a. deleting the entry in item 12a on her DD Form 214;
b. adding the entry, "1983 04 13" in item 12a on her DD Form 214;
c. deleting the entry in item 12c on her DD Form 214;
d. adding the entry, "0020 00 18" in item 12c on her DD Form 214;
e. deleting the entry in item 12d on her DD Form 214;
f. adding the entry, "0000 00 00" in item 12d on her DD Form 214;
g. deleting the entry, "PERIOD OF DELAYED ENTRY PROGRAM: 19830401-
19830417" in item 18 on her DD Form 214; and
h. adding the entry, "PERIOD OF DELAYED ENTRY PROGRAM: 19830401-
19830412" in item 18 on her DD Form 214.
