THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of item 12b (Date Entered Active Duty (AD) This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 30 September 2014 to show 4 February 1993 vice 12 April 2006.
2. The applicant states 12 April 2006 was the date of her indefinite reenlistment in the Regular Army (RA) and not the initial date she entered AD. She enlisted in the U.S. Army Reserve (USAR) on 26 November 1991 and she was separated from the USAR on 3 December 1992, for which she has a DD Form 214. She has all her enlistment contracts as well as a DA Form 1506 (Statement of Service - For Computation of Length of Service for Pay Purposes) showing her initial entry as well as all of her combined service.
3. The applicant provides two DD Forms 214, DA Form 1506, seven DD Forms 4 (Enlistment/Reenlistment Document - Armed Forces of the United States), two DA Forms 1695 (Oath of Extension or Enlistment), DA Form 3286 (Statements for Enlistment),DA Form 3340 (Request for Reenlistment or Extension in the RA), and seven Army Leave and Earnings Statements.
CONSIDERATION OF EVIDENCE:
1. The applicant's records show she enlisted in the USAR on 26 November 1991 for a period of 8 years. On 28 July 1992, she entered active duty for training (ADT). She was honorably released from ADT on 3 December 1992 to the control of her USAR unit. The DD Form 214 she was issued for this period of service shows she completed 4 months and 6 days of creditable active service. On 3 February 1993, she was discharged from the USAR for the purpose of enlistment in the RA.
2. The applicant provides, and her record contains, a DD Form 4, dated 4 February 1993, that shows she enlisted in the RA on that date for a period of 3 years.
3. She provides, and her record contains, DD Forms 4 dated:
* 13 November 1995, showing she reenlisted in the RA on that date for a period of 4 years
* 19 January 1999, showing she reenlisted in the RA on that date for a period of 3 years
* 3 July 2001, showing she reenlisted in the RA on that date for a period of 4 years
* 2 August 2004, showing she reenlisted in the RA on that date for a period of 2 years
* 12 April 2006, showing she reenlisted in the RA on that date for an indefinite period
4. She provides a DD Form 1506, dated 15 October 2013, wherein it shows she completed:
* 8 months and 2 days of inactive service in the USAR from 26 November 1991 to 27 July 1992
* 4 months and 6 days of AD service as a member of the USAR from 28 July to 3 December 1992
* 2 months of inactive service in the USAR from 4 December 1992 to 3 February 1993 (for a total of 10 months and 2 days of inactive service)
* AD service in the RA from 4 February 1993 to the present
5. She was retired on 30 September 2014 by reason of sufficient service for retirement and she was transferred to the Retired List on 1 October 2014.
She completed 21 years, 7 months, and 27 days of AD service from 4 February 1993 through 30 September 2014.
6. The DD Form 214 she was issued for this period of service shows the following entries in:
* item 12a - 2006--04--12
* item 12b (Separation Date This Period) - 2014--09--30
* item 12c (Net Active Service This Period) - 0008--05--19
* item 12d (Total Prior Active Service) - 0013--06--14
* item 12e (Total Prior Inactive Service) - 0000--09--22
DISCUSSION AND CONCLUSIONS:
The evidence of record confirms the applicant entered initial AD in the RA on 4 February 1993 and served continuously until 30 September 2014 when she was retired and placed on the Retired List on 1 October 2014. She completed 21 years, 7 months, and 27 days of creditable active service during this period. Her record also confirms she had 4 months and 6 days of prior active service and 10 months and 2 days of prior inactive service. It is obvious the DD Form 214 she was issued for this period of service contains many administrative errors that should now be corrected.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 for the period ending 30 September 2014 by:
* deleting the entry in item 12a and replacing it with the entry - 1993--02--04
* deleting the entry in item 12c and replacing it with the entry - 0021--07--27
* deleting the entry in item 12d and replacing it with the entry - 0000--04--06
* deleting the entry in item 12e and replacing it with the entry - 0000--10--02
