      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) be corrected to show the following entries:
item 12d: 3 years; item 12e: 25 years; item 12f: 1 year, 4 months, 18 days;
item 13: 2nd award of Army Achievement Medal (AAM), Humanitarian Service
Medal (HSM), Army Reserve Components Overseas Training Ribbon (ARCOTR) (3rd
Award), Noncommissioned Officer Professional Development Ribbon (NCOPDR)
with Numeral "3", and Army Overseas Ribbon; item 14: U.S. Army Military
Police School (USAMPS), Observer Controller Course (OC), the Basic
Noncommissioned Officer Course (BNCOC)and the Advanced Noncommissioned
Officer Course (ANCOC).

2.  The applicant states that his unit failed to have records at the
demobilization site to verify previous service and it resulted in an
incomplete DD Form 214.

3.  The applicant provides a DD Form 214 with a separation date of 17
August 2003; a DD Form 214 with an effective date of 6 October 1977; two
NGB Form 22 (Report of Separation and Record of Service) from the Oklahoma
Army National Guard (OKARNG) with effective dates of 1 January 1990 and 31
October 1995; two Army Achievement Medal Certificates dated 8 March 1998
and 12 July 1998; two OKARNG Form 38 dated 22 November 1992 and 26
September 1995; State of Oklahoma Order 41-170 and 215-105; a certificate
of participation dated 18 March 1989; three DA Forms 1059 (Service School
Academic Evaluation Report); a USAMPS Certificate; a Basic Noncommissioned
Officer Course Certificate; an Advanced Noncommissioned Officer Course
Certificate; a copy of his DA Form 2-1 (Personnel Qualification Record); a
copy of his Enlistment/Reenlistment Document, date 1 Novemeber1995 and a
copy of an AHRC Form 249--2-E (Chronological Statement of Retirement
Points).

CONSIDERATION OF EVIDENCE:

1.  The DD Form 214 with an effective date of 6 October 1977 shows active
service of 3 years.  This form also shows foreign service of 1 year, 4
months and 18 days.

2.  The NGB Form 22 with an effective date of 31 October 1995 shows the
applicant's record of service included 16 years 8 months and 3 days of
Reserve Component service.

3.  The Enlistment/Reenlistment Document  and AHRC Form 249--2-E
(Chronological Statement of Retirement Points) shows he enlisted in the
United State Army Reserve (USAR) on 1 November 1995 and was separated on 2
March 2004, an additional 8 years 5 months and 1 day of Reserve Component
service.

4.  The AAM Certificates that the applicant provided shows he was awarded
this award twice for two periods of meritorious service.

5.  Item 9 (Awards, Decorations and Campaigns) of the applicant's DA Form 2-
1 shows the following:

      a.  award of the HSM
      b.  award of the ARCOTR 3
      c.  award of the Noncommissioned Officer Professional Development
Ribbon with numeral "3"
      d.  award of the Army Overseas Service Ribbon

6.  The applicant’s records contain a certificate of completion from the
U.S. Army Military Police School, dated 13 February 1975; a certificate for
each of the Basic and Advanced Noncommissioned Officer Courses, dated 13
January 1985 and 16 May 1987 respectively; and a certificate of training
showing completion of the 75th Division Lanes Observer Controller (OC)
Course.

7.  Army Regulation 635-5 (Separation Documents) provides that item 13
(Decorations, Medal, Badges, Citations and Campaign Ribbons Awarded or
Authorized) of the DD Form 214 will contain awards and decorations for all
periods of service.

DISCUSSION AND CONCLUSIONS:

1.  Records show that the applicant served 3 years of active duty.
Therefore item 12d his DD Form 214 should be corrected to show three years.

2.  Records show that the applicant served a total of 25 years 1 month and
4 days of Reserve Component service.  Therefore, his records should be
corrected to show this service.

3.  Records show that the applicant performed 1 year, 4 months and 18 days
of foreign service.  Therefore, item 12f of his DD Form 214 should be
corrected to show this service.

4.  Records show that the applicant was awarded two AAMs.  Therefore, his
DD Form 214 should be corrected to show these awards.
5.  Records show that the applicant was awarded the HSM.  Therefore, his DD
Form 214 should be corrected to show this award.

6.  Records show that the applicant was awarded the ARCOTR (3rd award).
Therefore his DD Form 214 should be corrected to show this award.

7.  Records show that the applicant was awarded the NCOPDR with numeral
"3". Therefore his DD Form 214 should be corrected to show this award.

8.  Records show that the applicant was awarded the Army Overseas Ribbon.
Therefore his DD Form 214 should be corrected to show this award.

Records show the applicant successfully completed the U.S. Army Military
Police School, the Basic and Advanced Noncommissioned Officer Courses, and
the 75th Division Lanes Observer Controller (OC) Course.  Therefore, his DD
Form 214 should be corrected to show these military education courses.

9.  In accordance with regulations military education will only be entered
on the DD Form 214 if it was completed during the period covered by the DD
Form 214. Therefore, the applicant's military education should not be
entered on the DD Form 214 with a separation date of 17 August 2003.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for relief.  As a result, the Board recommends
that all Department of the Army records of the individual concerned be
corrected to show the applicant served 3 years of active service; served 25
years, 1 month, and 4 days of inactive service; served 1 year, 4 months,
and 18 days of foreign service; award of the Army Achievement Medal with 1
oak leaf cluster; award of the Humanitarian Service Award; award of the
Army Reserve Components Overseas Training Ribbon (3rd Award); award of the
Noncommissioned Officer Professional Development Ribbon with numeral "3",
award of the Army Overseas Ribbon and successful completion of the U.S.
Army Military Police School, the Basic and Advanced Noncommissioned Officer
Courses,  and the 75th Division Lanes Observer Controller (OC) Course.
