The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of
Release or Discharge from Active Duty) for the period ending 30 June 2003
to show all of her total prior active service and total prior inactive
service.
2. The applicant states that her DD Form 214 does not show all of her
total active service. She attended the Officer Basic Course from 17
October 1984 through 19 December 1984. She states that she was discharged
from this period of active duty on 8 August 1988 after completing 4 years
of active service. She came back on active duty on 19 March 1992 and was
discharged on 30 June 2003 for an interdepartmental transfer to the Navy.
Her DD Form 214 needs to reflect the correct active duty and inactive duty
so she may receive all pay and entitlements due to her.
3. The applicant provides two DD Forms 214; a letter from the Chief of the
Medical Service Corps; two appointment letters as a Reserve commissioned
officer; two Oaths of Office (DA Form 71); orders awarding her the Army
Commendation Medal; her Leave and Earnings Statement for the period
covering 1 through 30 June 2003; and her Officer Record Brief.
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed in the Reserve as a Medical Service Corps
commissioned officer on 9 August 1984.
2. She was ordered to active duty on 13 October 1984 for a 3-year active
duty commitment and was released from active duty on 8 August 1988. She
was transferred to the U.S. Army Reserve Control Group on the date
following.
3. The applicant was issued a DD Form 214 for the period ending 8 August
1988 which shows she completed 3 years, 9 months and 26 days total active
service and 2 months and 4 days total prior inactive service.
4. She remained a member of the U.S. Army Reserve and was a Health
Professions Scholarship Program (HPSP) student at medical school from
August 1988 to May 1992.
5. The applicant accepted appointment in the Reserve as a Medical Corps
commissioned officer on 3 June 1992.
6. The applicant's Officer Record Brief shows her entry on active duty
(EAD) current tour date as 15 June 1992. It shows her basic active service
date (BASD) as 19 August 1989.
7. She was promoted to lieutenant colonel on 17 July 2002.
8. The applicant was honorably released from active duty on 30 June 2003
by reason of intradepartmental transfer. She is currently serving full-
time support duty in the U.S. Navy.
9. She was issued a DD Form 214 for the period ending 30 June 2003 which
shows she completed 13 years, 10 months and 12 days total active service
during the period covered by the report. Item 12a (Date Entered Active
Duty This Period) shows she entered active duty on 19 August 1989.
10. Item 12d and item 12e on the applicant's DD Form 214 for the period
ending 30 June 2003 does not show she completed any prior active service or
prior inactive service.
11. Army Regulation 635-5 (Separation Documents) governs the preparation
of the DD Form 214. It states that the DD Form 214 is a synopsis of the
Soldier's most recent period of continuous active duty. It provides a
brief, clear-cut record of active Army service at the time of release from
active duty, retirement or discharge. In pertinent part it directs that
the total prior active service and total prior inactive service less lost
time will be taken from previously issued DD Forms 214 and the total amount
will be entered in items 12d and 12e.
DISCUSSION AND CONCLUSIONS:
1. The applicant was appointed a Reserve commissioned officer on 9 August
1984.
2. She served on active duty from 13 October 1984 through 8 August 1988
and was issued a DD Form 214 which reflects she completed 3 years, 9 months
and 26 days total active military service and 2 months and 4 days total
prior inactive service. She remained in the U.S. Army Reserve while
attending medical school and was reappointed in the Medical Corps on 3 June
1992.
3. She was issued a DD Form 214 for the period ending 30 June 2003 which
shows she entered active duty on 19 August 1989. However, there is no
evidence of record available which indicates she entered active duty on
19 August 1989 and this date appears to have been erroneously taken from
her BASD. Her Officer Record Brief shows her EAD current tour date as
15 June 1992.
4. Although the applicant completed 3 years, 9 months and 26 days total
prior active military service and 2 months and 4 days total prior inactive
service, her DD Form 214 for the period ending 30 June 2003 does not
reflect any prior active service or prior inactive service.
5. It appears that administrative errors have occurred in this case
regarding the applicant's prior active service and prior inactive service.
Therefore, it would be appropriate to recalculate the applicant's active
duty service to determine her total prior active service and total prior
inactive service as of her DD Form 214 for the period ending 30 June 2003.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that the DD
Form 214 for the period ending 30 June 2003 of the individual concerned be
corrected:
a. by amending item 12a to show she entered active duty this period
on 15 June 1992;
b. by amending item 12c to show she completed 11 years and 15 days
net active service this period;
c. by amending item 12d to show she completed 3 years, 9 months and
26 days total prior active service; and
d. by amending item 12e to show she completed 2 months and 4 days
total prior inactive service.
