      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) be corrected to show he is a combat veteran.

2.  The applicant states that there is nothing on his DD Form 214 to show
he is a combat veteran.

3.  The applicant provides his DD Form 214 for the period ending 19
December 2003; his deployment orders; a memorandum approving award of the
Joint Service Achievement Medal (JSAM); JSAM orders; and a JSAM award
certificate.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s military records are not available to the Board.  This
case is being considered using reconstructed records which primarily
consist of the documents provided by him and his performance fiche
(containing his officer evaluation reports).

2.  After having had prior service, the applicant entered active duty on 5
June 2000.  He was assigned to the 1st Battalion, 377th Field Artillery,
Fort Bragg, NC around January 2001.

3.  The applicant was deployed, apparently as an individual, to the U. S.
Central Command area of responsibility in support of Operation Enduring
Freedom        on or about 21 July 2002.  His officer evaluation report for
the period ending        3 February 2003 shows he was in Afghanistan at
that date.

4.  Combined/Joint Task Force 180 Operation Enduring Freedom, Bagram
Airfield, Afghanistan Permanent Orders J-065-011 dated 17 December 2002
awarded the applicant the JSAM for the period 9 August 2002 to 3 February
2003.

5.  The applicant was honorably discharged with disability severance pay on
    19 December 2003.  His DD Form 214 for this period does not list the
JSAM in item 13 (Decorations, Medals, Badges, Citations and Campaign
Ribbons Awarded or Authorized) and does not show he was deployed to
Afghanistan in item 18 (Remarks).

6.  Army Regulation 635-5 prescribes the separation documents prepared for
soldiers upon retirement, discharge, or release from active military
service or control of the Army.  It establishes standardized policy for the
preparation of the DD Form 214.  In pertinent part it states that, for an
active duty soldier deployed with his or her unit, the statement "Service
in (Name of country deployed) from (inclusive dates)" will be entered in
item 18.

DISCUSSION AND CONCLUSIONS:

1.  It appears the applicant was deployed to the U. S. Central Command area
   of responsibility in July 2002, arrived in Afghanistan on 9 August 2002,
and departed Afghanistan on 3 February 2003.  Although it appears he
deployed as an individual and not as a member of his unit, it would be
equitable to amend item 18 of his DD Form 214 for the period ending 19
December 2003, as an exception to policy, to show he was deployed to
Afghanistan.

2.  The applicant's DD Form 214 for the period ending 19 December 2003
should also be amended to add the JSAM.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that the DD
Form 214 for the period ending 19 December 2003 of the individual concerned
be corrected by amending item 18 to show he deployed to Afghanistan from
        9 August 2002 through 3 February 2003 and to add the Joint Service
Achievement Medal.
