APPLICANT REQUESTS: That his records be corrected by changing his social security account number (SSAN) and his first and middle name.
APPLICANT STATES: That his SSAN should read ____, and not
_____, and his name should read ____, and not _____. In support of his request the applicant submits a copy of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge), and Form SSA-2458 (Report of Confidential Social Security Benefit Information) from the Social Security Administration.
EVIDENCE OF RECORD: The applicant's military records show:
He was inducted into the Army of the United States on 24 July 1969, for a period of 2 years. He served in Vietnam from February 1970 to March 1971.
The applicant was inducted into the Army under the name of _____. Documents in the applicant’s records, to include his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) were signed by the applicant as “______.”
At the time of the applicant’s induction, the Army was using military service number (MSN) and not social security account number. The applicant was assigned the MSN _____.
Effective 1 July 1969, the Department of Defense directed that the military service number would be replaced by the member’s social security account number.
The applicant was notified by the U.S. Army Administration Center, St. Louis, Missouri, that his personnel records failed to reveal his SSAN, and requested that one be provided to them. An annotation on a request for his SSAN shows that ____ had been coded into the system as his SSAN.
On 14 March 1971, the applicant was honorably discharged under the provisions of Army Regulation 635-200, for early separation of overseas returnees. His DD Form 214 Indicates he had 1 year, 7 months and 3 days of creditable service, and was separated under the name of _____, and the SSAN of ______. The applicant authenticated his DD Form 214 with his signature.
On 29 March 1972, after the sixth request, the applicant submitted his SSAN of
_____, to the U.S. Army Reserve Components Personnel and Administration Center. His SSAN was coded into the system on 3 August 1972.
DISCUSSION: Considering all the evidence, allegations, and information presented by the applicant, together with the evidence of record, applicable law and regulations, it is concluded:
1. Evidence shows the applicant was inducted into the Army under the name of ______, and used this name through out his military service. There is no evidence or documentation that shows he used the name of ______.
2. Evidence shows that the applicant’s military service number was used during his active duty military service. After a sixth request, the applicant submitted a social security account number, that was coded into the system on 3 August 1972. The applicant submits no documentation to show that he had been issued a social security account number prior to his separation from active duty.
3. In order to justify correction of a military record the applicant must show to the satisfaction of the Board, or it must otherwise satisfactorily appear, that the record is in error or unjust. The applicant has failed to submit evidence that would satisfy that requirement.
4. In view of the foregoing, there is no basis for granting the applicant's request.
DETERMINATION: The applicant has failed to submit sufficient relevant evidence to demonstrate the existence of probable error or injustice.
