2. The applicant requests, in effect, that his date of birth, social security number and rank be changed on his DD Form 214, Armed Forces of the United States Report of Transfer or Discharge.
3. The applicant states, in effect, that the date of birth, social security number, and rank shown on his DD Form 214 are incorrect and they must be corrected for social security purposes.
4. The applicant submitted a copy of his Social Security Account Number Card and a copy of his birth certificate issued by the Louisiana State Department of Health, Division of Public Health Statistics, on about 19 March 1987, in support of his application.
5. The applicant’s military records show that he enlisted in the rank and pay grade Private, E-1, in the Louisiana Army National Guard on 10 November 1965 for 6 years. The applicant was ordered to active duty for training on 9 June 1967. The applicant's orders to active duty for training show the applicant's rank and pay grade as Private, E-2, in the standard name line.
6. The record shows that the applicant was honorably released from active duty for training on 8 October 1967 in the rank and pay grade PV2, E-2, and was provided a DD Form 214.
7. Block 3 (Social Security Number) of the applicant's DD Form 214 shows the number *** / 44 / 160. The first three digits on the Social Security Account Number Card and the first three digits in Block 3 are the same numbers. The remaining numbers are *** / 68 / 8271.
8. Block 9 (Date of Birth) shown the applicant's date of birth as 26 August 1943. The date of birth recorded on the applicant's birth certificate is 26 August 1944.
9. All available documents in the applicant's service records which require entry of a date of birth show the date, 26 August 1943.
10. There is no evidence that the applicant was promoted to a rank and pay grade higher than that shown on his DD Form 214.
11. On 28 September 1967, just 11 days before he was released from active duty for training, the applicant completed a DA Form 3082-R, Statement of Medical Condition. On this form, in the block requiring his "Last Name – First Name – Middle Initial, Grade & Service Number," he entered, "E-2."
CONCLUSIONS:
1. The Board noted the applicant’s contentions that the date of birth, Social Security Account Number and the rank entered on the DD Form 214 issued him on his release from active duty for training are incorrect; however, it finds that only the date of birth and Social Security Account Number are in error.
2. The Social Security Account Number shown in Block 3 of the DD Form 214 is one digit short and the numbers are the same as the last five digits of the applicants Selective Service Number that is shown in Block 16 of the DD Form 214. The incorrect Social Security Account Number should be corrected at this time.
3. The date of birth recorded in Block 9 of the DD Form 214 is incorrect and should be corrected accordingly. There is no advantage to be gained by the applicant by making this correction to his DD Form 214; therefore, the incorrect date of birth should be corrected at this time.
4. The rank and pay grade shown in Block 5a, PVT E-2 (P), on the applicant's DD Form 214 are correct. No change is recommended nor required.
5. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected by amending the DD Form 214 of the individual concerned by:
a. correcting the entry in Block 3 to show the applicant's correct Social Security Account Number to read, *** / 68 / 8271, and
b. correcting the applicant's date of birth in Block 9 of his DD Form 214 to 26 August 1944 from 26 August 1943.
2. That so much of the application as is in excess of the foregoing be denied.
