2. The applicant requests that item 17c (Date of Entry) of his DD Form 214 (Report of Transfer or Discharge) be corrected to show that he entered active duty on "5 December 1966" instead of "6 December 1972."
3. The applicant states that his date of entry is incorrect on his DD Form 214 and that the date shown is after his discharge. In support of his application, he submits a copy of his DD Form 214, copy of his Minnesota Application for Vietnam Bonus, and a copy of his immunization certificate.
4. The applicant’s military records show he enlisted on 6 December 1966, as a helicopter repairman, for a period of 3 years. He continued to serve until he was honorably discharged on 16 November 1969.
5. The applicant’s records contain a copy of DD Form 4 (Enlistment Record – Armed Forces of the United States) which shows that the applicant enlisted on 6 December 1966.
6. Item 17a of the applicant's DD Form 214 shows the entry "6 DEC 72"
(6 December 1972), which is incorrect.
7. The applicant provided a copy of his Minnesota Application for Vietnam Bonus, dated 28 August 1973, which shows his period of active duty from 5 December 1966 to 16 November 1969.
8. The applicant's immunization certificate shows that he was immunized on 7 December 1966.
CONCLUSIONS:
1. The evidence of record clearly shows that the applicant enlisted on "6 December 1966" instead of "6 December 1972." Therefore, he is entitled to correction of item 17c of his DD Form 214 to show the entry "6 DEC 66" instead of the entry "6 DEC 72."
2. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case for the individual concerned be corrected by showing in item 17a (Date of Entry) of his DD Form 214 the entry "6 DEC 66."
2. That so much of the application as is in excess of the foregoing be denied.
