2. The applicant requests, in effect, that his military records be changed to reflect his correct social security number (SSN).
3. The applicant states, in effect, that the SSN listed in his separation document (DD Form 214) is incorrect. In support of his application, he submits copies of the following documents: a letter, dated 20 May 2002, from the Assistant District Manager, Social Security Administration, Cape Girardeau, Missouri; his social security card; a medical insurance card; a selective service card; and his DD Form 214.
4. The applicant’s military records show that he was inducted into the Army of the United States on 13 January 1966, and that he continuously served on active duty for 1 year, 9 months, and 4 days, until being honorably separated on
16 October 1967.
5. A review of the applicant’s Official Military Personnel File (OMPF) shows that a service number was used as the primary identification number at the time the applicant entered active duty, and this was the identification number listed in his induction paperwork, the service record created on him upon his entry on active duty, and in the orders and documents prepared and published on him throughout his tenure on active duty.
6. The DD Form 214 issued to the applicant on the date of his separation from the Army lists a SSN in addition to his service number. The listed SSN differs from the one recorded in the applicant’s social security card.
7. In support of his application, the applicant provides a letter from the Social Security Administration, Assistant District Manager, Cape Girardeau, Missouri. This letter confirms that the applicant received his original SSN and social security card in June 1962, and there is no record of another SSN for the applicant. This letter lists the applicant’s verified SSN and confirms it is the same one issued to the applicant in 1962, and that he has used ever since.
CONCLUSIONS:
1. The Board notes the applicant’s contention that the SSN listed in item
3 (Social Security Number) of his DD Form 214 is incorrect, and it finds this claim has merit.
2. The evidence of records confirms that his service number was recorded in the military documents prepared and published throughout his active duty service and that the only military record document that contains a SSN is the DD Form 214 issued to him upon his separation.
3. The applicant provides independent evidence that confirms that he was issued a SSN in 1962, and that this is the only SSN he has used since. The applicant’s SSN that has been verified by the Social Security Administration and which is contained in his social security card differs from the one listed in the applicant’s separation document. Therefore, the Board finds that the SSN listed in Item 3 (Social Security Number) of the applicant’s DD Form 214 was in error, and it concludes that it would be appropriate to correct this error at this time.
4. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by changing the SSN of the individual concerned to match the verified SSN contained in his social security card; and by providing him a corrected separation document that reflects this correct SSN.
