2. The applicant requests, in effect, that his military records be changed to show his correct social security number (SSN).
3. The applicant states, in effect, that the SSN listed on his separation document (DD Form 214) is incorrect. In support of his application, he submits a copy of his social security card, and a statement from the Social Security Administration, Batavia Ohio, which confirms that his social security card serves as an official verification of his SSN.
4. The applicant’s military records show that he entered the Army on 5 February 1965. He was trained and served in military occupational specialty 13D (Artillery Rocket Crewman), and the highest rank he attained while serving on active duty was specialist four/E-4 (SP4).

A review of the applicant’s official military personnel file (OMPF) confirms that a service number was used as the primary identification number at the time he entered the Army and was the identification number listed in his induction paperwork. It was also the identification number entered in the service record that was created on him upon his entry on active duty, and in all the orders and documents published throughout his tenure on active duty that are filed in his Military Personnel Records Jacket (MPRJ).

However, in addition to the applicant’s service number, a SSN that matches the one contained in the social security card was listed in parenthesis in the following Special Court Martial (SPCM) Order published on the applicant: SPCM Order 6, dated 15 February 1968, published by Headquarters, 184th Ordnance Battalion, APO San Francisco 96238.
7. On 25 May 1968, the applicant was honorably released from active duty after completing 3 years and 21 days of active military service. The DD Form 214 issued to him on the date of his separation contains a SSN that is different from the one that is listed in both his social security card and in the SPCM Order referred to in the preceding paragraph.
8. In support of his application, the applicant provided a social security card and a verification from the Social Security Administration, Batavia Ohio. This documentation confirms that the applicant’s current verified SSN is consistent with the one recorded in SPCM Order Number 6, which was published on him in 1968.
CONCLUSIONS:
1. The Board notes the applicant’s contention that the SSN listed in Item
3 (Social Security Number) of his DD Form 214 is incorrect, and it finds this claim has merit.
2. The evidence of records confirms that the applicant’s current verified SSN is consistent with the one used in a SPCM Order published on him in 1968, which is different than the one recorded in Item 3 of his DD Form 214.
3. In addition, the applicant has submitted documentation from the Social Security Administration that verifies his correct SSN, which is consistent with the one recorded in the SPCM published on him in 1968. Therefore, the Board finds that the SSN listed in Item 3 (Social Security Number) of the applicant’s DD Form 214 was in error, and it concludes that it would be appropriate to correct this error at this time.
4. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by changing the SSN of the individual concerned to match the verified SSN contained in his social security card and that is recorded in SPCM Order Number 6, dated 15 February 1968; and by providing him a corrected separation document reflects this change.
