2. The applicant requests that his Certificate of Release or Discharge from Active Duty, DD Form 214, for the period ending 14 June 1997 be corrected to show he had 2 years, 9 months, and 26 days of prior active duty.
3. The applicant states that his DD Form 214 incorrectly shows he had 10 years and 27 days of prior active duty. He provides his DD Form 214 for the period ending 5 June 1986 as supporting evidence.
4. The applicant’s military records from his initial period of service only are available. They show that he was born on 13 September 1964. He entered the Delayed Entry Program on 12 October 1982. He enlisted in the Regular Army on 10 August 1983. He was released from active duty on 5 June 1986 after completing 2 years, 9 months, and 26 days of net active service that period. There is no evidence of record to show he had any active duty while in the U. S. Army Reserve Control Group (Reinforcement). (A summary of retirement points was not found on the applicant. Only a detailed points history was available but it begins with retirement point start date 15 April 2000.)
5. The applicant’s DD Form 214 for the period ending 14 June 1997 shows that he reenlisted in the Regular Army on 21 March 1990 and was discharged on 14 June 1997 upon completing his required active service. That DD Form 214, item 12d shows that he completed a total of 10 years and 27 days of prior active service.
6. Apparently, the applicant enlisted in the U. S. Army Reserve on 15 April 2000.
CONCLUSIONS:
1. There appears to be an error on the applicant’s DD Form 214 for the period ending 14 June 1997, item 12d. The evidence of record shows that he had only 2 years, 9 months, and 26 days of prior active service.
2. It appears that the applicant enlisted in the U. S. Army Reserve on 15 April 2000. He is encouraged to contact the U. S. Army Reserve Personnel Command to ensure his retirement points history is accurate.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That the applicant’s DD Form 214 for the period ending 14 June 1997, item 12d be amended to show he had 2 years, 9 months, and 26 days of total prior active service.

The applicant states his DD Form 214 is incorrect because it does not show his prior active and inactive service. After completing his required active service, the applicant was honorably released from active duty on 17 March 1997. Paragraph 010301, DODFMR states, in pertinent part, that commissioned officers with over 4 years of prior active service as an enlisted member are entitled to additional basic pay at the O-1E rates.

The evidence of record shows the applicant enlisted in the U.S. Army Reserve under the DEP on 16 May 1981 and was discharged on 8 September 1981, which is a period of 3 months and 23 days. Although there is no evidence of record which shows the applicant was issued a DD Form 214 for his period of active duty service from 9 September 1981 to 30 June 1983, he should have been issued one. As a result, the Board recommends that the DD Form 214 of the individual concerned be corrected by: a....

The applicant requests correction of military records as stated in the application to the Board and as restated herein. When he later requested a copy of the DD Form 214, the available Certificate of Release or Discharge from Active Duty, DD Form 214, made a poor copy so an NA Form 13808 was prepared for him. This error was administratively corrected and now shows he initially entered service on 1 December 1987 with a final retirement year ending date of 30 November 1995 (the date he was...

The applicant requests correction of military records as stated in the application to the Board and as restated herein. Those were the same dates that the applicant submitted via the provided DD Form 214 for which she was requesting credit for increased special pay rates. In any case, the form shows only that the applicant completed 74 days of active duty (not over 3 years) prior to her entry on active duty in July 1999.
