2. The applicant requests reconsideration of his previous application to correct his military records by showing the last two digits of his Social Security Number (SSN) as 65 not 66 and by showing that he completed a General Educational Development (GED) certificate.
3. The applicant states, in effect, that he needs to have these two items corrected as they are causing him employment and other administrative problems. He asks that his Report of Separation or Discharge (DD Form 214) be corrected to show these changes.
4. Incorporated herein by reference are military records which were summarized in the decisional document prepared to reflect the Board's previous consideration of the case (AC97-08104) on 15 October 1997.
5. The applicant submits a copy of a Report of Educational Achievement Through The United States Armed Forces Institute (USAFI) Transcript and a Social Security Administration (SSA) printout.
6. The applicant’s submissions are new evidence that requires Board consideration.
7. The information on the SSA printout matches the information on the applicant’s enlistment documents as to his full name, date and place of birth, father’s name and mother’s maiden name. There is no SSN on any of the entry documents and all other official documents in his records show his SSN as ending in 66.
8. The applicant submits a copy of a USAFI transcript that shows his satisfactory completion of the GED over the period of June 1972 to May 1974.
CONCLUSIONS:
1. The applicant’s records show that he served under the SSN ending in 66 during his entire period of service. However, the Board concludes that the SSA document shows that this is an incorrect SSN for the applicant. Therefore, it is appropriate to correct the applicant’s records to reflect the correct final digits as 65.
2. The applicant’s records show that he completed only 11 years of formal education prior to or during his period of military service, therefore the entry at item 20 on his DD Form 214 is correct.
3. However, the Board concurs with the applicant that his records should show that he obtained his GED. The appropriate location for this notation would be under item 27, Remarks, on his DD Form 214.
4. In view of the foregoing findings and conclusions, correcting the applicant’s records as recommended below would correct an error and rectify an injustice.
RECOMMENDATION:
That the individual concerned be issue a DD Form 215 to show, in block 27 (Remarks) of his DD Form 214, “Individuals correct SSN is ***-**-**65” and GED, 1974”.
