2. The applicant requests that his Certificate of Honorable Discharge be corrected to show the correct initial for his middle name. He also requests that his DD Form 214 (Report of Separation from the Armed Forces of the United States) be corrected to show that he was discharged on 28 November 1953.
3. The applicant states, in effect, that his DD Form 214 should show that he was discharged on 28 November 1953, which is the same date that he was transferred to the United States Army Reserve (USAR).
4. The applicant's military records were lost or destroyed in the National Personnel Records Center fire of 1973. Information herein was limited to those documents that were submitted by the applicant and consist of a DD Form 214, a Certificate of Honorable Discharge; and a copy of Special Orders Number 235, Headquarters, VI United States Army Corps (Reserve), Fort Benjamin Harrison, Indianapolis, Indiana, dated 30 November 1959.
5. The applicant's DD Form 214 shows that he was inducted into the Army of the United States on 29 November 1951 and he served until he was honorably discharged on 17 November 1953. He had served 1 year, 11 months and
19 days of active military service. His name is correctly shown on his DD Form 214, with his middle name being "Alphonse."
6. The applicant's Certificate of Honorable Discharge shows that he was transferred to the USAR on 18 November 1953. His middle initial is incorrectly shown as "I" instead of "A."
7. The Orders that the applicant submitted show that he was discharged from the USAR on 28 November 1959 upon completion of his 6 year USAR service obligation. His Honorable Discharge Certificate was issued to him on the same date.
CONCLUSIONS:
1. The applicant's separation documents correctly show that he was discharged on 17 November 1953 and he was transferred to the USAR on 18 November 1953.
2. The applicant's Certificate of Honorable Discharge should be corrected to show that his middle initial is "A" for Alphonse.
3. In view of the foregoing, the applicant’s records should be corrected, but only as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected by issuing to the individual concerned a new Certificate of Honorable Discharge showing that the correct initial for his middle name is "A."
2. That so much of the application as is in excess of the foregoing be denied.

Della R. Trimble | |Member | The Board considered the following evidence: Exhibit A - Application for correction of military records. Without the applicant’s medical records, or other form of evidence, it is not possible to determine if his DD Form 214 is in error. As a result, the Board recommends that the individual’s DD Form 214 dated 1 July 1953 be corrected to show his middle name as “Dorne.” 2.
