2. The applicant requests that the service number on his DD Form 214, Report of Separation, be corrected.
3. The applicant states that the Army service number on his DD Form 214 is incorrect. He contends that the last three digits of his service number should be 245. In support of his request, he furnished copies of orders showing his correct service number and a copy of his DD Form 214.
4. The applicant’s military records were lost or destroyed in the 1973 fire at the National Personnel Records Center. Information herein is derived from the document provided by the applicant.
5. His DD Form 214 shows that he was ordered to active duty from the Arkansas National Guard on 21 August 1950. His place of entry into active service was listed as Boonville, Arkansas, and his service number is listed as NG_ _ _ _ _ 254. He completed 1 year, 10 months and 29 days service and was honorably released and returned to the National Guard on 19 July 1952.
6. Orders issued by the 24th Evacuation Hospital on 20 December 1950, show that the applicant was placed on 6 days leave. The last three digits of his service number on these orders are 245.
CONCLUSIONS:
1. Identifying Information shown on the applicant’s DD Form 214, i. e. home address and the prefix and the first five numbers of his service number are consistent with the information shown on the orders he furnished and his current application. The last three digits of his service number on the DD Form 214, however, are different. He contends they should be 245, instead of 254. Based on the information available, it appears there is a typographical error on the DD Form 214 that should be corrected.
2. Accordingly, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected
by changing the last three digits of the service number on the DD Form 214 of the individual concerned to 245.
