         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that his report of separation (DD Form 214) be corrected to reflect his award of the NATO Medal, completion of the Special Forces (SF) Military Freefall Jumpmaster Course and the Department of Defense (DOD) High Risk Survival Course.

3. The applicant states, in effect, that he was authorized the award of the NATO Medal, that he completed the 3-week SF Military Freefall Jumpmaster Course in 1995 and the 1-week DOD High Risk Survival Course in 1995. However, they are not included on his DD Form 214. In support of his application he submits copies of training certificates and his authorization for award of the NATO Medal.

4. The applicant’s military records show that he enlisted on 23 November 1977, that he served in a variety of airborne and SF assignments, was promoted to the pay grade of E-8 on 1 August 1994.

5. His records show that he completed the 1-week DOD High Risk Survival Course in 1993, that he completed the 3-week SF Freefall Jumpmaster Course in 1995 and that he was authorized the award of the NATO Medal on 7 January 1998.

6. The applicant was honorably released from active duty (REFRAD) on 31 March 1998 and was placed on the Retired List effective 1 April 1998. He had served 20 years, 4 months and 8 days of total active service. His DD Form 214,
issued on the date of his REFRAD, fails to indicate that he had completed the courses or that he was awarded the NATO Medal.

CONCLUSIONS:

1. The evidence of record as well as the evidence submitted by the applicant clearly show that he completed the two courses in question and that he was authorized the award of the NATO Medal. However, it appears that an administrative oversight resulted in their omission from his DD Form 214 at the time of his REFRAD.

2. Accordingly, the Board finds that in the interest of justice his records should be corrected to reflect his completion of the two courses and to add his award of the NATO Medal.

3. In view of the foregoing, the applicant’s records should be corrected as recommended below.




RECOMMENDATION: That all of the Department of the Army records related to this case be corrected by showing that the applicant completed the 1-week DOD High Risk Survival Course in 1993, that he completed the 3-week SF Freefall Jumpmaster Course in 1995 and that he was authorized the award of the NATO Medal.
