3. On 22 September 1969, he was inducted into the Army of the United States.
4. During the period 29 September-13 November 1969, he was assigned to a special training unit; 14 November 1969- 16 January 1970, BCT; and, 17 January-12 April 1970, AIT (MOS 13A, Field Artillery Basic). 7. AR 600-8-22, Military Awards, indicates, in pertinent part, that:
a. the PH is awarded for wounds caused by hostile action and requiring medical treatment with a record being made and
b. bronze service stars are authorized for each designated campaign. From 1 May to 30 June 1970 was the DA Sanctuary Counteroffensive Campaign and from 1 July 1970 to 30 June 1971 was the UN Counteroffensive Phase VII Campaign.
CONCLUSIONS:
1. The evidence of record shows that he is entitled to have his record corrected to show his BCT and AIT completion.
2. There is no evidence to support his claim for the PH, battle star, or additional qualification on various weapons.
3. The evidence does show that he should be credited with one additional campaign.
4. In view of the foregoing, his record should be corrected, but only as shown below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected by showing he completed BCT, AIT and that he is credited with one additional bronze service star.
2. That so much of the application as is in excess of the foregoing be denied.

The evidence of record confirms that the applicant was awarded the PH and BSM and that these awards were announced in official orders. The evidence of record also clearly shows that both the hospital and the applicant's unit issued orders awarding him the PH based on being wounded in action on 30 May 1970. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending Item 24 of the DD Form 214 by deleting the current list of...

The PH is not included in the awards listed on any of these separation documents, and the applicant authenticated each of them with his signature on the date they were issued. Further, the PH is not included in the list of awards contained on any the applicant's four separation documents, the last of which was issued upon his retirement in 1983, and there is no indication that the applicant pursued his claim of entitlement to the PH at anytime in the almost 13 years he remained on active...

The applicants Enlisted Qualification Record (DA Form 20) shows he served in the RVN from 12 January 1970 through 6 December 1970. The applicant provides a PH Certificate which shows that on 20 July 1970, the 91st Evacuation Hospital awarded him the PH for wounds he received in action on 19 July 1970. Therefore, it would be appropriate to add his award of the PH to his record and separation document at this time.

The evidence of record confirms that the applicant was awarded the PH and the BSM for three separate periods, and that these awards were announced in official orders. As a result, Item 24 of the applicants separation document should be corrected to add the PH and to show his entitlement to the BSM with 2nd OLC to denote three awards instead of two awards of the BSM. As a result, it would be appropriate to award the applicant the first award of the AGCM, for his qualifying period of...
