2.  The applicant requests correction of his military records to show on his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending on 1 February 1986 that the place of entry into active duty was San Antonio, Texas, instead of Fort Sill, Oklahoma.

3.  At the time of the applicant’s enlistment in the U.S. Army Reserve Control Group (Reserve Officers’ Training Corps, his home of record was shown as San Antonio, Texas.

4.  A Report of Medical Examination, dated 12 June 1973, shows that the applicant’s place of birth and home address were in San Antonio.

5.  On 17 August 1973, he was appointed as a Reserve commissioned officer, in the rank of second lieutenant, at St. Mary’s University, San Antonio.

6.  On 8 January 1974, the applicant took the Oath of Office for the Texas Army National Guard, as a second lieutenant, in San Antonio.

7.  Orders issued by Fort Sill, dated 12 March 1974, indicate that the applicant was ordered to active duty from the Texas Army National Guard, in the rank of second lieutenant, as a Reserve officer of the Army effective 14 March 1974; that his home of record was in San Antonio; and that the place that he entered active duty/ordered to active duty was Fort Sill.

8.  Special Orders, dated 20 March 1974, indicate that the applicant was separated from the Texas Army National Guard effective 13 March 1974, and that his permanent address was in San Antonio, Texas.

9.  The applicant’s DD Form 214 for the period ending on 1 February 1986 indicates that he was honorably released from active duty, in the rank of captain, and that his place of entry into active duty was Fort Sill.



9.  Army Regulation 635-5 prescribes the separation documents which are prepared for individuals upon retirement, discharge, or release from active military service or control of the Army.  Table 2-1 provides the 
DD Form 214 preparation instructions.  For item 6 (Place of Entry Into Active Duty), it indicated that the city and state from which the member last entered on active duty from civilian life would be entered.

CONCLUSIONS:

1.  The evidence of record indicates that the applicant was born in San Antonio; that he accepted a Reserve commissioned officer appointment in San Antonio; that he took the Oath of Office for the Texas Army National Guard in San Antonio; and that his home of record was San Antonio when he was ordered to active duty as a Reserve officer of the Army.

2.  Although the orders, which ordered him to active duty, show the place that he entered active duty/ordered to active duty was Fort Sill, the evidence shows that the city and state from which he last entered on active duty from civilian life was San Antonio, Texas.

3.  In view of the foregoing, it would be appropriate to correct the applicant’s military records as recommended below.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by showing that the individual concerned entered on active duty on 17 February 1974 in San Antonio, Texas, instead of Fort Sill, Oklahoma.
