APPLICANT REQUESTS THAT:
His DD Form 214, Certificate of Release or Discharge from Active
Duty, be corrected to reflect the following:
a) Block 1, Name to read XXX (Will be administratively
corrected).
b) Block 13, add the “Kuwait Liberation Medal for Kuwait and
Saudi Arabia (Will be administratively corrected).
c) Block, 13, add the Air Force Outstanding Unit Award with
Valor.
d) Block 18, add “Vietnam Era Veteran.”

APPLICANT CONTENDS THAT:
1. He served honorably on active duty for more than 20 years.
Part of his active duty time occurred between 5 Aug 64 and 7 May
75; therefore, by definition he is a “Vietnam Era Veteran.”
2. Between 8 Aug 90 and 14 Mar 91, he served with the 363rd
Tactical Fighter Wing Provisional in support of Operations
DESERT SHIELD/STORM.
In support of his request, the applicant provides copies of his
DD Form 214, notarized Order and Certificate of Name Change and
Amendment of Birth Record, Desert Shield/Storm Tour Data
Certification sheet and other documentation associated with his
request.

STATEMENT OF FACTS:
The applicant served on active duty from 27 Nov 74 to 31 Dec 94.
He is credited with 4 years, 9 months and 19 days of Foreign
Service. The applicant was deployed to “Site V” from 10 Aug
90 to 14 Mar 91, in support of Operations DESERT SHIED/STORM.
The applicant’s DD Form 214 verifies he was awarded the KLM;
however, it does not reflect whether he was awarded the KLM-
Kuwait or KLM-Saudi Arabia.
On 2 Apr 12, AFPC/DPSSRP notified the applicant that in
accordance with (IAW) AFI 36-2608, Military Personnel Records
System the source document used for a name change must be either
the original document or a certified/notarized copy (raised seal
and/or original signature).
On 13 Sep 12, AFPC/DPSOR notified the applicant that IAW AFI 36-
3202, Separation Documents they cannot add the statement
“Vietnam Era Veteran” to his DD Form 214; however, his Kuwait
Liberation Medal has been amended to read Kuwait Liberation
Medal-Kuwait (KLM-K) and will be administratively corrected.
On 22 Oct 12, AFPC/DPSIRP notified the applicant that his
request to update his name in the Personnel Data System will be
administratively corrected.
On 28 Nov 12, AFPC/DPSID reevaluated the applicant’s case due to
his rebuttal comments to the Air Force advisory. DPSID states
that based on the applicant’s deployment dates, combined with
the Southwest Asia Service Medal (SWASM) campaign awards, he
would be awarded the KLM-Saudi Arabia (KLM-SA) and the Air
Force Expeditionary Service Ribbon with Gold Border (AFESR
w/GB).

AIR FORCE EVALUATION:
AFPC/DPSIRP recommends denial of the applicant’s request to
change his name in his military personnel records. DPSIRP
states the applicant has not provided the requested source
documentation.

AFPC/DPSID recommends denial of the applicant’s request for
award of the Air Force Outstanding Unit Award (AFOUA) with
Valor. DPSID states they cannot verify any units the applicant
served with during Operations DESERT STORM/SHIELD that were
awarded the AFOUA with Valor. DPSID states that AFPC/DPA has
been unable to verify a location for “Site V”; however, his
record contains an Air Force Form 7, Airman Military Record
awarding him the KLM for service in the area of responsibility
(AOR) from 9 Aug 90 to 14 Mar 91.
His DD Form 214 also reflects award of the SWASM with two bronze
service stars (SWASM w/2BSS); therefore, based on his service
dates in the AOR, DPSID verified the applicant’s entitlement to
award of the KLM-K for his service in support of Operations
DESERT STORM/SHIELD.

APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION:
By letter dated 25 Oct 12, the applicant reiterates his original
contentions. He states he complied with instructions for the
name change; AFPC/DPAPP was confused and thought he was asking
for boots-on-ground; he should receive the Kuwait Liberation
Medal for Saudi Arabia and Kuwait as he requests, and the “V”
device was overlooked in his records.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of error or injustice to warrant award
of the AFOUA w/Valor or to add “Vietnam Era Veteran” to his DD
Form 214. We took notice of the applicant’s complete submission
in judging the merits of the case; however, we agree with the
opinions and recommendations of the Air Force offices of primary
responsibility (OPR) and adopt their rationale as the basis for
our conclusion the applicant has not been the victim of an error
or injustice. We note that DPSIRP has made the necessary
changes to effect his name change. In addition, AFPC/DPSID
verified the applicant’s entitlement to the AFESR w/GB, KLM-K
and KLM-SA and will administratively correct his record to
reflect
these
aside
administrative corrections noted above, we find no basis to
recommend granting further relief in this case.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that
the application was denied without a personal appearance; and
that the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered
with this application.
