APPLICANT REQUESTS THAT:
Her records be corrected to reflect she was awarded the Meritorious
Unit Award (MUA) and the Air Force Outstanding Unit Award (AFOUA) with
eight Oak Leaf Clusters (OLCs).

The applicant believes she is entitled to the medals.

STATEMENT OF FACTS:
The applicant entered the Air Force on 14 August 1984.
The applicant’s DD Form 214 reflects she was awarded the Air Force
Commendation Medal (AFCM) with one OLC, Air Force Achievement Medal
(AFAM) with one OLC, Meritorious Service Medal with three OLCs,
Humanitarian Service Medal, National Defense Service Medal (NDSM) with
Bronze Star (BS), Air Force Training Ribbon, Air Force Longevity
Service Award (AFLSA) with four OLCs, Global War on Terrorism Service
Medal, Air Force Outstanding Unit Award (AFOUA) with seven OLCs.
The applicant’s Report of Individual Person (RIP) reflects her date
arrive station to the 92 Air Refueling Wing, Fairchild AFB was 15 July
2002.
The applicant’s Officer Performance Reports (OPRs) for the periods
8 April 1999 through 7 April 2000, 8 April 2000 through 7 April 2001,
8 April 2001 through 7 April 2002 reflects the applicant was assigned
to the 55th Medical Operations Squadron,
Offutt AFB. The OPRs for the periods 8 April 2002 through 7 April
2003 and 8 April 2003 through 7 April 2004 reflect she was assigned to
the 92nd Air Refueling Wing, Fairchild AFB.
The SECAF in March 2004 approved the MUA to recognize organizations
for their outstanding achievement or service in direct support of
combat operations.
The applicant retired on 1 October 2005 in the grade of lieutenant
colonel. She served 21 years, 1 month and 17 days of active duty
service.

AIR FORCE EVALUATION:
AFPC/DPPPR states the applicant was assigned to the 55th Medical
Operations Squadron, Offutt AFB. This unit received the MUA for the
period 1 June 2002 to 31 May 2004. The applicant had already departed
the unit and was assigned to the 92nd Medical Operations Squadron,
Fairchild AFB.
They reviewed the applicant’s military records and were unable to
verify her entitlement to the MUA and an additional OLC for the AFOUA.
AFPC/DPPPR further states the applicant has not provided
documentation to substantiate her request. They were unable to verify
any additional unit awards including verifying whether the applicant’s
last duty assignment was presented the unit award. They recommend the
applicant’s request for the MUA and an additional OLC for the AFOUA be
denied.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
The applicant reviewed the Air Force evaluation and states Special
Order GA-033 awarded the MUA to the 55th Medical Operations Squadron
for the period of 1 June 2002 through 31 May 2004. She was attached
to the 55th Medical Operations Squadron from 28 May 1999 through 14
July 2002.
Per the Permanent Change of Station orders her transfer effective date
(TED) was July 2002. She physically departed the 55th Medical
Operations Squadron on 6 July 2002 and reported to the 92nd Medical
Operations Squadron on 15 July 2002.
She further states the dates referencing the time period she was
assigned to the 92nd Medical Operations Squadron by HQ AFPC/DPPPR are
incorrect. She was assigned to the 92nd Medical Operations
Squadron from 15 July 2002 until her final day of active duty
30 September 2005. The dates cited by HQ AFPC/DPPPR are the rating
periods on her OPRs which only illustrate the unit of assignment at
the OPR closeout dates and who had sufficient number of days of
supervision to rate her. She further states these dates do not
reflect when she was assigned to the unit and should not be used for
this determination.
Regarding her AFOUA, she reviewed her dates of assignment and the
award dates. She concurs with HQ AFPC/DPPPR that her DD Form 214
accurately reflects her entitlement to the AFOUA.
She believes she should receive the MUA based on the fact she was
attached to the 55th Medical Operations Squadron from 28 May 1999 to
14 July 2002.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law
or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of an error or an injustice to warrant partial relief. The
applicant’s request for award of the MUA and an additional OLC for her
AFOUA is duly noted. The office of primary responsibility upon
reviewing the applicant’s records indicated she received the AFOUA
with seven OLCs and the applicant after reviewing her records
concurred with the OPR’s findings for award of the AFOUA with seven
OLCs. In regard to award of the MUA, the applicant was assigned to
the 55th Medical Operations Squadron which was awarded the MUA for the
period 1 June 2002 through 31 May 2004. The applicant provided clear
and persuasive evidence showing she was assigned to unit after the
award was issued. Therefore, we recommend her records be corrected to
the extent indicated below.

THE BOARD DIRECTS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that she was awarded the
Meritorious Unit Award for outstanding achievement or service in
direct support of combat operations for the period 1 June 2002 through
31 May 2004.
