APPLICANT REQUESTS THAT:

Her records be corrected to reflect her married name and a new DD Form
214 be issued.

APPLICANT CONTENDS THAT:

Her name has changed.

In support of her request, the applicant provides a copy  of  DD  Form
214, Certificate of Release or Discharge from Active Duty.

Applicant’s complete submission, with attachment, is at Exhibit A.

STATEMENT OF FACTS:

The applicant enlisted in the Regular Air Force on 29 July  1975,  and
was retired in the grade of technical sergeant on 1 August 1995.   She
served 20 years and 2 days of total active service.

AIR FORCE EVALUATION:

AFPC/DPSFCM  recommends  denial.   AFI  36-2608,  Military   Personnel
Records System, Table A7.2, Rule 1, states that if a member submits  a
name change request due to marriage, then the member must furnish  the
original   or   certified   copy   of   the   marriage    certificate.
Correspondence was sent to the member on 23 December 2003,  requesting
the required document.   Additionally,  a  message  was  left  on  her
answering machine on  28 January  2004,  requesting  she  furnish  the
required documentation.  As of this date, she has made no contact with
their office.  Therefore, this application should be denied.

The DPSFCM evaluation is at Exhibit B.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to the  applicant  on
27 Feb 04, for review and comment within 30 days.  As of this date, no
response has been received by this office.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing  law
or regulations.

2.  The application was not  timely  filed;  however,  it  is  in  the
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented  to  demonstrate
the existence of an error or injustice.  The applicant  was  asked  on
several occasions to furnish the original or  certified  copy  of  her
marriage certificate to support her claim, but she failed  to  respond
to  those  requests.   Since  she  did  not  provide  the   additional
information requested in order to sufficiently evaluate her claim,  it
is our opinion that no basis exists to grant the relief sought in this
application.  Accordingly, the applicant's request  is  not  favorably
considered.

THE BOARD DETERMINES THAT:

The  applicant  be  notified  that  the  evidence  presented  did  not
demonstrate the existence of material error  or  injustice;  that  the
application was denied without a personal  appearance;  and  that  the
application will only be reconsidered upon  the  submission  of  newly
discovered relevant evidence not considered with this application.
