951127: Mental status evaluation cleared applicant for separation.
 960104: Unit commander notified the applicant of initiation of separation
action under the provisions of Chapter 7, AR 635-200, fraudulent
entry. He was advised of his rights.
 960104: Unit commander recommended separation with a general under honorable
conditions discharge.
960105: Applicant acknowledged notification, consulted legal counsel and
submitted a statement in his own behalf.

honorable conditions discharge.
960109: Physical examination cleared applicant for separation.
