1. Summary of facts and circumstances concerning discharge:
         880804: Mental status evaluation cleared applicant for separation.
         880907: Unit commander notified the applicant of initiation of separation
                  action under the provisions of Chapter 14, AR 635-200, for
                  misconduct-pattern of misconduct. He was advised of his rights.
         880908: Applicant consulted legal counsel, requested a hearing by a board of
                  officers. Applicant also requested a conditional waiver, and did
                  not submit a statement in his own behalf.
         880907: Unit commander recommended separation from the Service.
         880922: Intermediate commander recommended approval with an under other than
                  honorable conditions discharge.
         880922: Senior intermediate commander recommended approval with an under
                  other than honorable conditions discharge.
         881002: Applicant again consulted legal counsel and waived a board of
                  officers.
         881004: Staff judge advocate determined the separation action be legally
                  sufficient.
