THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his deployment to Iraq from 2004 to 2005.
2. The applicant states this deployment is not listed on his DD Form 214.
3. The applicant provides copies of a DA Form 638 (Recommendation For Award) and his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant's military records show he enlisted in the Regular Army on 6 January 1998 and he held military occupational specialty 11C (indirect fire infantryman).
2. He provided a copy of a DA Form 638 showing he was recommended for award of the Army Commendation Medal for outstanding service during Operation Iraqi Freedom III from 6 October 2004 through 1 October 2005. Permanent Orders Number 164-07 approved the award on 13 June 2005.
3. He served continuously on active duty until he was honorably discharged on 4 March 2014. He was credited with completing 16 years, 1 month, and 29 days of net active service this period . Item 18 (Remarks) of his DD Form 214 lists the entry, "Service in Iraq from 20070809-20081013."
4. The Defense Finance and Accounting Services (DFAS) verified the applicant received hostile fire and imminent danger pay for service in Kuwait/Iraq from 8 October 2004 through 23 September 2005, a period of 11 months, and 15 days.
5. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states the DD Form 214 would be prepared for all personal at the time of their retirement, discharge, or release from active duty. The regulation states item 18 for active duty Soldiers deployed with their units will list the entry, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD YYYYMMDD)"
DISCUSSION AND CONCLUSIONS:
DFAS verified during the applicant's period of active duty from January 1998 to March 2014 he also served in Kuwait/Iraq from 8 October 2004 through 23 September 2005. Therefore, item 18 of his DD Form 214 should be corrected to show this service.
